Introduction SARS-CoV-2 affects part of the innate immune response and activates an inflammatory cascade stimulating the release of cytokines and chemokines, particularly within the lung. Indeed, the inflammatory response during COVID-19 is likely the cause for the development of acute respiratory distress syndrome (ARDS). Patients with mild symptoms also show significant changes on pulmonary CT-scan suggestive of severe inflammatory involvement. Hypothesis The overall hypothesis is that HBO 2 is safe and reduces the inflammatory response in COVID-19 pneumonitis by attenuation of the innate immune system, increase hypoxia tolerance and thereby prevent organ failure and reduce mortality. Evaluation of the hypothesis HBO 2 is used in clinical practice to treat inflammatory conditions but has not been scientifically evaluated for COVID-19. Experimental and empirical data suggests that HBO 2 may reduce inflammatory response in COVID-19. However, there are concerns regarding pulmonary safety in patients with pre-existing viral pneumonitis. Empirical data Anecdotes from “compassionate use” and two published case reports show promising results. Consequences of the hypothesis and discussion Small prospective clinical trials are on the way and we are conducting a randomized clinical trial.

BACKGROUND Hemoglobin‐Based Oxygen Carriers (HBOCs) can act as an “oxygen bridge” in acute severe anemia when transfusion is indicated, but not possible. We present data on 10 Expanded Access (EA) patients treated with high cumulative doses of Hemopure (HBOC‐201), to assess the ability of HBOC‐201 to safely treat life threatening anemia in situations where high volumes of product were administered over an extended period of time. STUDY DESIGN AND METHODS Inclusion in this study required that the patient receive at least 10 units of HBOC‐201 between 2014 and 2017 under the FDA‐sanctioned EA program. Depending on a patient's geographical location, treatment with HBOC‐201 was obtained through either a single patient emergency Investigational New Drug (IND) application, or an intermediate size population IND. Of the 41 patients who were treated during this period, 10 patients received 10 or more units of the product. Data were obtained from medical records. RESULTS Treatments with HBOC‐201 started within 24 hours of signing consent and were administered at an average rate of 1.99 (SD 0.17) units per day over a mean of 8.2 days (SD 2.9), during which patients received on average 16.2 units (SD 5.7 units) of HBOC‐201. The median pre‐treatment nadir corpuscular hemoglobin (Hb) concentration was 3.3 (SD 0.9) g/dL and post‐treatment Hemoglobin was 7.3 (SD 1.7) g/dL. Common side effects included methemoglobinemia, gastrointestinal symptoms, and hypertension. However, no product‐related serious adverse events (SAEs) were noted. All patients survived. CONCLUSIONS Administration of HBOC‐201 over an extended period is a feasible and safe oxygen bridge for severely anemic patients who cannot be transfused with RBC.

Background: Oral candidiasis generally caused by Candida albicans with prevalence of 30-50%. C. Albicans proliferation in the oral cavity was canducted by decreasing immune system of the host. Lymphocytes activation in immune system happened when there is a contact between host cell with cell wall of C. albicans as a result of C. albicans antigen. One of therapy to suppress the Candida infection sistemically is by giving hyperbaric oxygen therapy. Purpose: To determine the effects of giving hyperbaric oxygen therapy to the amount of lymphocytes in oral candidiasis immunosuppressed models. Methods: This research was post test only control group design. We used 12 wistar rats which were divided into 3 groups(n=4/3): K-(normal/ healthy wistar rats), K+(oral candidiasis immunosuppressed wistar rats), P1(oral candidiasis immunosuppressed wistar rats which were given hyperbaric oxygen therapy 5 days). K+ and P1 groups were immunosuppressed by giving dexamethasone 0,5mg/day/rat orally for 14 days, added with tetrasiklin 1 mg/day/rat. On the 3rd day immunosuppression, the rats were inducted with C.albicans smeared in the dorsum linguae once every two days for 12 days. Results: The data were analyzed with one way Anova test and showed significant difference among groups (p<0,05). To show the difference between each groups we used LSD test and showed significant difference (p<0,05) between K+ compared with P1, and K+ compared with K-. Conclusion: Hyperbaric oxygen therapy gives effect to the amount of lymphocytes in oral candidiasis immunosuppressed models.
