Objective: To compare the live birth rate, multiple pregnancy rate, and obstetric outcomes of elective single and double embryo transfers.Design: Case series with internal comparisons.Setting: University affiliated hospital, Hong Kong. Participants:Between October 2009 and December 2011, 206 women underwent their first in-vitro fertilisation cycle. Elective single embryo transfer was offered to women who were aged 35 years or below, and had endometrial thickness of 8 mm or more and at least two embryos of good quality. Main outcome measures:Live birth rate, multiple birth rate, and obstetric outcomes.Results: Among the 206 eligible women, 74 underwent an elective single embryo transfer and 132 a double embryo transfer. The live birth rate was comparable in the two groups, being 39.2% in the elective single embryo transfer group and 43.2% in the double embryo transfer group, while the multiple pregnancy rate was significantly lower in the elective single embryo transfer group than the double embryo transfer group (6.9% vs 40.4%; P<0.001). Purpose : To find an effective method to select embryos from ICSI with better chromosomal status when preimplantation-genetic-diagnosis (PGD) is not applied. Methods : Several morpholological evaluations were done in same embryos. Embryos from ICSI were classified on day 1 according to pronuclear-nucleoli arrangement. On day 3, classification was done according to number, fragmentation, size and shape of cells. In some patients, embryos exhibiting good quality on day 3 (at least six regular blastomeres with cell fragmentation lower than 20%) were also submitted to PGD, irrespective to pronuclearnucleoli morphology. Results : Forty-two per cent of normally fertilized embryos showed pronuclear-nucleoli-goodmorphology; from those, 86% were also classified as good quality on day 3. Good-quality embryos submitted to PGD have shown lower chromosomal abnormality rates when also classified as pronuclear-nucleoli-good-morphology. Conclusions : Pronuclear-nucleoli morphology seems to be correlated with PGD results. This criterion may prove useful for pre-select embryos with normal chromosomal package when PGD is not applied.

A exigência de legalização e controle das novas formas de atuação a adotar no âmbito das aplicações das tecnologias de reprodução assistida é potenciada, entre outros aspectos, pela percepção dos respectivos riscos e incertezas, diretos e indiretos, quer para a sociedade, quer para o indivíduo 1 .Em Portugal, a delegação nos profissionais médicos e investigadores e nas comissões de bioética, no Estado e seus agentes e no mercado e na indústria, afiguram-se como a principal resposta às problemáticas colocadas por essas tecnologias. Nesse contexto, torna-se essencial promover a pluralidade dos espaços de debate e de decisão, o que exige o desenvolvimento de um novo paradigma científico e social capaz de sobrepor a emancipação ao conhecimento e à regulação através, por exemplo, da aceitação e revalorização do caos, do controle precário das causas e da falta de controle sobre as conseqüên-cias das ações "científicas" 2 .No fundo, trata-se de compreender os usos sociais e as implicações das tecnologias de reprodução assistida a partir do reconhecimento da existência de um conjunto de processos heterogêneos e contingenciais, cujas interseções, complexidades e (des)regulações originam configurações singulares e incertas, como o aumento da capacidade de ação através de nexos de causalidade cada vez mais complexos e opacos,
