Retinal perfusion changes in radiation retinopathy

Abstract: The ionizing radiation seems to have an impact on the TRBF, SaO and SvO , clinically presenting similar to a rapidly developing diabetic retinopathy. The results show an altered retinal vascular physiology in patients with radiation-related retinopathy.

“…After irradiation of retina cell culture in vitro at doses ranging from 1 up to 2 Gy, which is the incidental dose received by the healthy retina per fraction when the standard treatment is delivered to the brain, at day two, an evident loss of cell viability and βIII-tubulin immunostaining was evident, while highlighting marked neuritic damage [2]. In eight patients who developed retinopathy following plaque radiation treatment for choroidal melanoma, the average total retinal blood flow (TRBF) was significantly lower, and retinal blood oxygen saturation indications were higher in the retinopathy eye, compared with the other, which indicates alterations of retinal vascularization, similar to a rapidly developing diabetic retinopathy [113].…”

“…In a small study of eight patients with radiation retinopathy after treatment for CM, a reduced blood flow with increased ArtSat and VenSat was identified in affected eyes. 13 No pretreatment values were reported however. An interesting comparison can be made with studies on retinal vasculature in treatment-naïve melanoma and nevi using OCTA.…”

“… 12 After treatment for CM, altered levels of retinal arterial and venous oxygenation were detected in patients with radiation retinopathy. 13 No pretreatment values were determined however, although such information might help to understand why some eyes develop radiation retinopathy and others do not. To the best of our knowledge, no studies on retinal oximetry in untreated melanoma or nevus eyes have as yet been reported.…”

“…High-dose brachytherapy, which uses iodine-125 b-ray sources (that can promote radiation retinopathy) to treat intraocular melanoma of the choroid, is accompanied by an increase in retinal oxygenation and reduced blood flow in the exposed eye (Rose et al 2018). These low-LET IRinduced changes in retinal microvasculature are a characteristic of diabetic retinopathy.…”

“…We found that hypoxic LECs and lenses have a relatively low radio-sensitivity, about twothirds of maximum OER; the retina, however, is more vulnerable to oxidative stress, as its well-oxygenated cells ($22 mmHg O 2 ) endure 90% maximum OER (Figures 3 and 5). Indeed, the vascular retina is prone to IR damage by the oxygen effect (Rose et al 2018).…”
