Background The purpose of this study was to summarize the clinical experience and 10 year follow-up results of laparoscopic assisted Soave procedure for the treatment of long-segment Hirschsprung disease (HD). Methods From January 2010 to February 2020, 106 children with long-segment HD participated in this study. The laparoscopic-assisted Soave procedure was performed for the treatment of long-segment HD. The follow-up time was two weeks, one month, and three months after the operation, and then every six months to one year. Results The operation was successful for all 106 children. All patients were discharged 5–7 days after the operation. The median time in surgery was 150 (100–190) minutes, and the median volume of bleeding was 6 (3–10) ml. The short-term postoperative daily defecation frequency was 4–11 times, 3–7 times within 6 months, and 2–3 times after 6–12 months. Postoperative complications included anastomotic leakage in two cases, perianal dermatitis in 13 cases, anastomotic stenosis in four cases, adhesive bowel obstruction in two cases, enterocolitis in 16 cases, soiling in 11 cases, and constipation recurrence in three cases. Conclusions The laparoscopic-assisted Soave procedure is a safe and effective surgical method for treating long-segment HD, and it causes little trauma or bleeding and has a fast postoperative recovery. Yet some complications may occur. Preoperative diagnosis, intraoperative and postoperative standardized processing can reduce the postoperative complications.

Purpose: This study aims to investigate risk factors of BE, ARM and acetylcholinesterase staining related to failure of treatments and diagnostic value of Hirschsprung’s disease (HSCR) or intractable constipation (IC). Methods: A single-center retrospective cohort study of children with constipation was conducted. All patients were followed up until significant improvement of defecation. Patients initially received general/conservative treatment or surgery. Upgraded treatments were adopted if the initial treatment failed. The risk factors including transitional zone, barium residue, absence of recto-anal inhibitory reflex (RAIR) and acetylcholinesterase staining. A p value <0.1 or <0.05 was considered to be statistically significant in Logistic regression. Chi-square test was used to calculate the sensitivity and specificity when analyzing the diagnostic value.Results: This study enrolled 4869 cases. Acetylcholinesterase staining +++ had the highest risk (OR, 177.613[95%CI 92.178~273.526]) leading to failure of general treatment, followed by absence of RAIR ((OR, 158.786[95%CI 92.178~273.526]) and severe barium residue (OR, 153.829[70.004~338.028]). The leading risk factors causing failure of conservative therapy were severe barium residue (OR, 15.530[95%CI 5.337~45.191]) and acetylcholinesterase staining +++ (OR, 13.242[6.657~26.337]). In children ≤3 years, the transitional zone, absence of RAIR and acetylcholinesterase staining ++/+++ had similar specificity (80.0%, 75.6% and 73.3%), whereas the acetylcholinesterase staining ++/+++ had the highest sensitivity (95.5%). The sensitivity was generally lower in children ＞ 3 years, however the specificity was similar between each age group.Conclusion: This real-world study validates that the performance of 3 examinations is of great significance in guiding upgrading treatments, predicting clinical outcomes and identifying HSCR and IC.Trial Number: NCT02216994 (registered on October 27, 2014)

Background Hirschsprung disease is commonly encountered by pediatric surgeons. Despite advances in the surgical management, these children may experience symptoms of bowel dysfunction throughout adulthood. Anorectal manometry may be used to assess post‐operative anorectal structure and function. This review aimed to consolidate and evaluate the literature pertaining to post‐operative findings of anorectal manometry in children with Hirschsprung disease. Purpose (1) Synthesize the available data regarding anorectal motility patterns in children following repair of Hirschsprung disease. (2) Evaluate the reported anorectal manometry protocols. Data Sources We performed a systematic review of four databases: Embase, MEDLINE, the Cochrane Library, and PubMed. Study Selection This systematic review was performed in accordance with Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA). Studies reporting results of post‐operative anorectal manometry in children with Hirschsprung disease were evaluated for inclusion. Results Twenty‐three studies satisfied inclusion criteria, with a combined cohort of 939 patients. Post‐operative anorectal manometry results were reported for 682 children. The majority of included studies were assessed as “poor quality.” Disparate manometry protocols, heterogeneous cohorts, and lack of standardized outcome assessments introduced a risk of outcome reporting bias, limited the comparability of results, and impeded clinical translation of findings. Conclusions This systematic review demonstrated the lack of high‐quality evidence underlying the current understanding of post‐operative anorectal motility in children with HD. There was little consistency in reported manometry outcomes between studies. In future work, emphasis must be placed on the application of standardized manometry protocols, cohort reporting, and patient outcome assessments.
