Purpose Unipolar and bipolar hemiarthroplasty (HA) are used to treat displaced femoral-neck fractures. However, which type is best for treating displaced femoral-neck fractures in elderly patients remains a subject for debate. Our aim was to review randomised controlled trials to establish which type provides superior clinical outcome for this patient population. Methods We searched PubMed, Embase and Cochrane Register of Controlled Trials databases and Web of Science for randomised controlled trials (RCTs) comparing unipolar with bipolar HA to treat femoral-neck fracture in the elderly. Risk ratios (RRs) and mean differences (MDs) from each trial were pooled using random-effects or fixed-effects models depending on study heterogeneity. Analysis was performed using RevMan5.2 from the Cochrane Collaboration. Results A total of 1,100 patients from nine studies were assessed in this meta-analysis. Results showed no significant differences in function score [MD=−0.14, 95% confidence interval (CI) −2.42-2.13], mortality (RR=0.97, 95% CI 0.65-1.46), dislocation (RR=1.33, 95 % CI 0.53-3.34), deep infection (RR=0.79, 95 % CI 0.35-1.79), acetabular erosion (RR=1.99, 95 % CI 0.61-6.52), operating time (MD=2.14, 95 % CI −9.85 to14.14), blood loss (MD=13.40, 95 % CI −49.60 to 76.39) and length of hospital stay (MD=0.12, 95 % CI −0.49to0.73) between unipolar and bipolar HA. Conclusions Unipolar and bipolar HA achieved similar clinical outcomes in patients with displaced femoral-neck fractures.
The intima media thickness (IMT) of the common carotid artery (CCA) can be used to predict the risk of atherosclerosis. Many image segmentation techniques have been used for IMT measurement. However, severe noise in the ultrasound image can lead to erroneous segmentation results. To improve the robustness to noise, a fully automatic method, based on an improved Otsu’s method and an adaptive wind-driven optimization technique, is proposed for estimating the IMT (denoted as “improved Otsu-AWDO”). First, an advanced despeckling filter, i.e., “ Nagare’s filter” is used to address the speckle noise in the carotid ultrasound images. Next, an improved fuzzy contrast method (IFC) is used to enhance the region of the intima media complex (IMC) in the blurred filtered images. Then, a new method is used for automatic extraction of the region of interest (ROI). Finally, the lumen intima interface and media adventitia interface are segmented from the IMC using improved Otsu-AWDO. Then, 156 B-mode longitudinal carotid ultrasound images of six different datasets are used to evaluate the performance of the automatic measurements. The results indicate that the absolute error of proposed method is only 10.1 ± 9.6 (mean ± std in μm). Moreover, the proposed method has a correlation coefficient as high as 0.9922, and a bias as low as 0.0007. From comparison with previous methods, we can conclude that the proposed method has strong robustness and can provide accurate IMT estimations.
Abstract. Gastrinoma is a gastrin-secreting tumor that is associated with Zollinger-Ellison syndrome. The majority of cases occur in the pancreas, followed by the duodenum. Early diagnosis is difficult due to the relative rarity of the tumor and the lack of specific symptoms. In the current study, a 68-year-old female patient presented at the Second Affiliated Hospital, Zhejiang University (Hangzhou, China) due to intermittent abdominal pain and watery diarrhea. The patient was treated by surgical resection and was pathologically diagnosed with a well-differentiated pancreatic neuroendocrine tumor (gastrinoma; grade 1). No evidence of recurrence was observed during 1 year of follow-up. Furthermore, a review of the Chinese literature was performed, which analyzed an additional 17 published cases of gastrinoma. The tumor size ranged between 0.5x0.5 cm and 7.5x6.3x5.1 cm. The pancreas was the most common site of occurrence, accounting for 72% (13/18) of cases, followed by the duodenum (28%; 5/18). The most common initial symptom was abdominal pain (89%; 16/18), followed by diarrhea (56%; 10/18). In 18 cases, including the present case and 17 previous cases, the level of gastrin ranged between 137 and 1,550 pg/ml (normal range, 5-100 pg/ml). Of the 17 previous cases, 11 patients underwent surgery and 6 patients received conservative therapy due to metastasis or patient choice. Overall, gastrinoma remains a rare disease. Complete removal of the lesion is the standard curative treatment and conservative treatment is only recommended for patients unsuitable for surgery or for those with widespread metastasis.
