Results of Radical Cystectomy and Pelvic Lymphadenectomy for Bladder Cancer with Pelvic Node Metastases

Abstract: Objectives: To study the results on survival of radical cystectomy and pelvic lymphadenectomy in bladder cancer with pelvic node metastases in a retrospective study. Patients and Methods: Of 248 patients with transitional cell carcinoma of the bladder treated by cystectomy with pelvic lymphadenectomy from 1970 to 1990,40 (16%; 37 males, 3 females) had pelvic nodal metastases. Fifteen patients had one positive lymph node (stage pN1) and 25 patients had more than one positive node (stage pN2). Preoperative pelvi… Show more

“…tosa, is a serious complication and occurs in 10% of reported cases. 10,11 The two earlier cases in our series showing local recurrence had severe urinary and bowel fistula. After having experienced these two cases, we introduced neoadjuvant therapy into the program and evaluated neoadjuvant therapy by WLCB and FNAB of the pelvic lymph nodes.…”

“…In the follow-up of 90 radical cystectomy patients, the 5-year survival was 22% and 38% in patients with and without lymph node involvement, respectively [8]. In another study performed on 248 radical cystectomy patients, lymph node involvement was found in 40 patients (16%), and the 5-year survival was 14 and 42% in groups with and without lymph node involvement, respectively [17]. These studies all indicate that lymph node involvement is not a good factor to use for prognosis.…”

“…The prognosis of the bladder cancer patients who have pelvic lymph node metastasis is poor. The 5‐year survival of these patients is reported as 7–30% 1–6 and recent works show 10‐year survival is 31–34%. 7,8 For the purpose of improving prognosis, fine needle aspiration biopsy (FNAB) on a diagnosis procedure for pelvic lymph node metastasis and multimodality therapy for positive FNAB cases were done.…”
