Results of AlloDerm use in abdominal hernia repair

Abstract: Early results in repair of abdominal hernia with AlloDerm appear to have a complication rate of 24%. Recurrence is the most common complication. Thinner AlloDerm use has better outcomes with less recurrence. Careful analysis regarding the technical aspects and presence of comorbidities may be explored to improve the present outcomes to prevent recurrences.

“…We also noted that, at 6 months, AlloDerm implants had become translucent, similar to what was reported by Gaertner et al [7]. Taken together, the apparent rapid remodeling and observed translucent appearance of the AlloDerm at 6 months may relate to the 20% recurrence rate of hernias following AlloDerm repairs reported by Misra et al [22] and to reports that AlloDerm does ''stretch'' over time [6,7].…”

“…This could account for the reported 20% hernia recurrence rate for this product observed by Misra et al [22]. CollaMend, on the other hand, exhibited evidence of encapsulation and poor ingrowth, similar to that observed by Chavarriaga et al [19] in their analysis of 18 patients undergoing abdominal wall reconstruction with this product.…”

“…Decellularized tissues have become widely used in regenerative medicine and some of them are even commercially available, mostly in the USA [1][2][3][4][5] (for example, human skin removed from cadavers which is supplied in dry form and commonly used in cases of third degree burn patients 1,2 ). The combination of physical, chemical and biological techniques provide the means for more efficient elimination of cellular and nuclear components, preservation of blood vessels' stroma and nerve pathways.…”

“…For example, thicker grafts are less likely to fully recellularize following implantation, a pathology that has been noted for both HAD and multi-ply SIS. 24,33 Delamination of laminated SIS has been noted following its use in hernia repair 34 and has been associated with intra-graft seroma between adjacent sheets of the laminated material. 24 Seroma formation within laminated grafts can be largely overcome by ensuring that the graft remains intact so that fluid cannot accumulate within the graft.…”
