Resolution of inverse psoriasis after treatment with levodopa for Parkinson's disease

Abstract: Inverse psoriasis is characterized by the development of erythematous shiny plaques at intertriginous areas of the body. It has a prevalence of 2% worldwide. The usefulness of levodopa in psoriasis was discovered in 1970 but nowadays it is not a standard therapy for this condition. A 74-year-old woman was diagnosed with Parkinson's disease subsequent to the development of extensive inverse psoriasis. The skin lesions were resistant to classical topical and systemic medications. “…A relationship between DA and the pathophysiology of psoriasis has been known since the 1970s. Thus, levodopa resulted in the resolution of inverse psoriasis when being used for Parkinson’s disease, possibly due to an increase in cyclic AMP 50. Moreover, bromocriptine, a strong D2R agonist, has been used in patients with psoriasis with different results on the clinical outcome 5153…”

“…Rojo Suárez et al described a case report of a patient with intertriginous psoriasis, resistant to standard therapy, and PD who was prescribed levodopa. First, they observed gradual improvement of skin and neurological condition and later relapse when the patient stopped taking her medications and again the resolution of skin lesions after reintroduction of levodopa (Rojo Suárez et al, 2017 ). Moreover, they discussed several similar studies.…”

“…We found four reports where resolution or improvement in psoriatic skin lesions was observed after administration of levodopa (Barbeau, 1970;Barbeau and Giroux, 1972;Giroux et al, 1972;Rojo Suárez et al, 2017). Levodopa is a dopaminergic drug used as a first-line treatment option for PD (Hayes, 2019).…”
