Renal cell carcinoma (RCC) is rare in children, its metastatic form is even rarer. At present there is no treatment protocol for children with RCC. This clinical observation presented the patient 10 year old with RCC metastases to the lungs. After radical surgery, after targeted sorafenib therapy the patient has a positive dynamics. At present his progression-free survival is 30 months.Key words: renal cell carcinoma, metastatic kidney cancer, children, targeted therapy, sorafenib Введение Рак почки является одной из наиболее актуальных проблем современной онкологии. В структуре онко-логической заболеваемости у взрослых пациентов почечно-клеточный рак (ПКР) составляет 2 % и за-нимает 3-е место среди злокачественных новообразо-ваний мочеполовой системы после опухолей предста-тельной железы и мочевого пузыря. ПКР обычно встречается в возрасте 50-70 лет [1]. У детей данная патология исключительно редка и составляет 1,8-6,3 % среди всех злокачественных опухолей почек, заболе-ваемость ПКР -в среднем 1 случай на 4 млн детского населения в год [1,2]. С возрастом заболеваемость ПКР увеличивается. По данным Японского общества детских хирургов, на ПКР приходится 1,4 % всех опу-холей почек у пациентов в возрасте до 4 лет, 15,2 % -5-9 лет и 52,6 % -10-15 лет [3,4]. Метастатическая форма ПКР у детей встречается редко, в литературе описано всего 18 ее случаев: M. Uchiyama и соавт. продемонстрировали лечение ПКР у 2 пациентов с регионарным метастатическим поражением в лим-фатические узлы; P. Indolfi и соавт. -3-летний опыт лечения ПКР с метастатическим поражением у 14 пациентов; R. S. Ismail-zade и соавт. -способ ле-чения ПКР с помощью гипертермии у 5 детей, среди которых 2 пациента имели метастатическое пораже-ние [3,[5][6][7].Данное заболевание в равной степени встречает-ся у детей обоих полов [8], в отличие от взрослых, у которых оно преобладает у мужчин. Наиболее рас-пространенные формы проявления ПКР у детей -макрогематурия и боль в боку, менее частые симпто-мы -синдром пальпируемой опухоли в брюшной полости, анемия и лихорадка [4,8]. Как правило, клинические проявления ПКР зависят от симптома-тики опухоли Вильмса у детей.Биологическое поведение и прогностические фак-торы ПКР у детей плохо изучены. До сих пор не суще-ствует протоколов лечения для детей с этой патологи-ей. С учетом устойчивости ПКР к химиотерапии и лучевой терапии хирургическое лечение, как и у взрослых, является основным методом, кото-рый приводит к выздоровлению, когда опухоль лока-лизована и полностью удалена. El riñón pediátrico es sitio frecuente de tumores que exhiben alteraciones cromosómicas característi-cas. El más común es el nefroblastoma o tumor de Wilms (TW) que se asocia con dos loci: 11p13 (WT1) y 11p15 (WT2 ó BWS), este último ligado también del síndrome de Beckwith-Wiedemann. Otros dos genes que parecen estar implicados son WT3 y WT4; además, dos anomalías específicas (adquisición 1q y deleción 22) se han correlacionado de manera independiente con un peor pronóstico en TW. Otras neoplasias con rearreglos cromosómicos, tales como los carcinomas renales (CRs), son mucho menos frecuentes en niños (entre el 1.8 y el 6.3% de todos los tumores renales malignos). Entre estos, se han identificado "CRs con translocación" que afectan el locus Xp11, siendo los dos tipos más importantes t(X;1), y t(X;17). El nefroma mesoblástico congénito (NMC) es un tumor renal de recién nacidos y lactantes. Los NMCs de la variedad celular se caracterizan por una translocación específica t(12;15)(p13;q25), misma que se encuentra también en los fibrosarcomas congénitos extrarenales, y que permite establecer una correspondencia genética entre estos dos tumores (NMC y fibrosarcoma congénito). Los tumores rabdoides (TR) del riñón son neoplasias muy infrecuentes y muy agresivas, que aparecen con una edad promedio de 11 meses. Al menos 50% de los TRs muestran anormalidades en el gen hSNF5/INI1, situado en el locus 22q11.2. Este gen probablemente está involucrado en la modulación transcripcional de otros genes, tales como el oncogen c-Myc, y de la vía de transducción de la proteína RB-retinoblastoma.Palabras clave: Citogenética. ABSTRACT RENAL TUMORS OF CHILDHOOD AND ADOLESCENCE ASSOCIATED WITH CHROMOSOMAL ANOMALIESThe pediatric kidney is a common site for tumors carrying specific chomosomal alterations. The most common of these is the nephroblastoma or Wilms tumor (WT), which is associated with anomalies in two loci: 11p13 and 11p15, the latter also linked to Beckwith-Wiedemann syndrome. Two other genes that seem to be implicated are WT3 and WT4. In addition, 1q gains or 22 deletions have been shown to independently be associated with a worst prognosis in WTs. Other neoplasms with chromosomal rearrangements, such as Renal Cell carcinomas (CRs) are much less frequent in children (between 1.8 and 6.3 % of all malignant renal tumors). Among these, the "translocation renal carcinomas" have been identified involving the locus Xp11 with two main types of translocations: t(X;1), and t(X;17). Congenital mesoblastic nephroma (NMC) is a renal tumor affecting newborns and young infants. NMCs of the cellular type feature a specific translocation t(12;15)(p13;q25), which is also present in congenital fibrosarcomas outside of the kidney. Clinicohistological characteristics of renal cell carcinoma in children: A multicentre study

Introduction: In this retrospective multicentre study, we compared the clinicohistological characteristics of renal cell carcinoma (RCC) between pediatric and adult patients. Methods: Data for patients who underwent radical or partial nephrectomy for RCC between 1988 and 2014 at multiple institutions were collected. Patients were divided into 2 groups according to age at diagnosis: pediatric patients (age ≤18 years) and adult patients (age ≥40 years). The groups were compared for clinical and pathologic variables, and survival analysis was performed. Results: The median follow-up period was 64 (range: 30-91) months for pediatric patients versus 44 (range: 19-59) months for adult patients (p = 0.026). Pediatric patients were mostly female (p = 0.003), had symptoms at presentation (p < 0.001), and had a high-stage tumour (p = 0.014) than adult patients. Among the symptomatic patients, gross hematuria was the most common symptom. The median tumour size was not different between groups. Regarding histologic types, pediatric patients had more papillary tumours (p < 0.001), more unclassified tumours (p < 0.001), and fewer clear cell carcinomas (p < 0.001). Five-year cancer-specific survival rates were 85% and 87.4% in pediatric and adult patients, respectively (log rank p = 0.901). Recurrence-free survival was better in adult patients, although this did not reach statistical significance (log rank p = 0.272). This study has several limitations, including its retrospective nature and the relatively small number of pediatric RCC cases. Conclusion: RCC in children is rare and is characterized by features that differ from those in adult RCC. Prognosis did not differ between groups.
