Reliability of the Modified TICI Score among Endovascular Neurosurgeons

Abstract: BACKGROUND AND PURPOSE:The modified TICI score is the benchmark for quantifying reperfusion after mechanical thrombectomy. There has been limited investigation into the reliability of this score. We aim to identify intra-rater and inter-rater reliability of the mTICI score among endovascular neurosurgeons. MATERIALS AND METHODS:Four independent endovascular neurosurgeons (raters) reviewed angiograms of 67 patients at 2 time points. k statistics assessed inter-and intrarater reliability and compared raters'-ver… Show more

“…Focusing on M1 occlusions, the DL system achieved an agreement of network-predicted and expertassigned mTICI scores in the range of published expert-based TICI scoring variability measures [15][16][17][18][19] for 2 independent DSA datasets from different centers. In addition, the dichotomous distinction between open (mTICI score ≥2b) and remaining states (mTICI score ≤2a) was highly reliable.…”

“…12,13 Despite its common use and the coarse distribution of the categories of the original TICI scale and suggested modifications (modified TICI [mTICI)], 14 expanded TICI 13 ), the inter- and intraobserver variability of TICI scoring is surprisingly high, with corresponding numbers varying from study to study (Table 1). 13,15–19…”

“…Furthermore, core laboratories use a limited number of experienced panelists, whereas site reads can be carried out by a wider array of interventionalists/readers with different degrees of expertise 6. It has been shown previously that the reliability of scoring and the degree of inter-rater agreement increase substantially with experience 25…”

“…Functional outcome at 90 days was assessed by board-certified vascular neurologists during a routinely scheduled clinical visit or by a study nurse certified in administering the mRS during a standardized telephone interview if the patient was unable to attend a clinic visit. Imaging variables, including mTICI score,20 were adjudicated by one interventionalist for each center.…”
