The levels of E-, P-and L-selectins in blood were studied during therapy in 42 patients with non-alcoholic fatty liver disease (NAFLD). Patients were divided into three groups. In the first group (17 patients), the hepatoprotective agent was combined with pentoxifylline. In the second group (10 patients) combination of hepatoprotective agent and metformin was applied. In the third group (15 patients), only a hepatoprotective agent (Phosphogliv forte) was prescribed. When using both combined therapy regimens, there could be a decrease in the concentration of E-and P-selectins in blood with normalization of P-selectin levels. Monotherapy with Phosphogliv forte reduced plasma levels of E-and P-selectins, which did not reach control values. In cases of biochemical remission of NAFLD (normalization of aspartic and alanine aminotransferases), a decrease in the concentration of E-and P-selectins in blood was noted. At the same time, levels of P-selectin reached normal values. In patients with preservation of increased activity of aspartic or alanine aminotransferases in dynamics of treatment, only parameters of P-selectin decreased. Thus, against the background of NAFLD therapy, there is a positive dynamics of soluble selectins, which is more pronounced in cases of combined treatment regimens.

Non-alcoholic fatty liver disease currently affects more than 30% of the population. Recent studies highlight the role of genetic polymorphisms in genes associated with fat catabolism and anabolism in the manifestation of this condition and its progression. The work analyzes foreign publications on the molecular and biochemical aspects of these polymorphisms, as well as works studying their effect on the state of the liver and markers of its pathology over the past 10 years. Thus, polymorphisms of the PNPLA3, MBOAT7, and TM6SF2, affecting the functionality of the proteins they express, lead to a change in the metabolism of fatty acids in the liver, which in turn leads to the development of NAFLD and its progression. Despite the fact that the contribution of the rs738409 polymorphism of the PNPLA3 gene is well described both in foreign and Russian articles, polymorphisms of the MBOAT7 and TM6SF2 genes and their effect on NAFLD, as well as the molecular biochemical mechanisms underlying it, have been studied much worse in foreign studies and are little mentioned in Russian ones. In addition, the issue of the severity of the influence of the above polymorphisms on populations of different ethnic and age groups requires additional research. This work attempts to systematize the available data on these issues.

Study of type 1 vascular adhesion molecules in patients with acute myocardial infarction with ST segment elevation at different body weights

Objective: to study the level of adhesion molecules in patients with STEMI with different BMI at the hospital stage and 12 months after the index event.Materials and methods: the study included 126 people with STEMI who had undergone PCI and 27 people in the control group. The analysis of the level of svcam‑1 in peripheral blood at the beginning of the disease and after 12 months was carried out, BMI and waist volume were measured. An assessment of the nature and frequency of complications after STEMI was performed.Results: the levels of biomarkers of the vascular endothelial adhesion molecule type 1 increase during the acute period of stemi, statistically signifi tly decrease, but remain increased by 3.5 times 12 months after the index event compared with the initial values. There was no association of VCAM‑1 with visceral obesity in the groups of our patients. Vascular endothelial adhesion molecules of type 1 increase in patients with a fatal outcome, as well as with an increase in the severity of OSN and CHF. Thus, VCAM 1 can be a predictor of an unfavorable outcome of AMI.Conclusions: the article presents the study of a marker of systemic inflammation VCAM‑11 in patients with STEMI with various types of obesity or BMI at the stage of hospitalization and outpatient follow‑up during the year. The determination of the VCAM‑1 level can be used to assess the intensity of the inflammatory process and the risk of adverse outcomes.
