Myositis Ossificans is characterized by the heterotopic formation of bone inside a muscle, tendons and ligaments. This pathology is traditionally classified into two groups: Myositis Ossificans Progressiva and Myositis Ossificans Traumatica. The pathogenesis of Myositis Ossificans Traumatica is still not well established. Because it is uncommon in the maxillofacial region, there are no consolidated treatment protocols. The purpose of this study is to present a case of a patient who evolved with Myositis Ossificans Traumatica of the masticatory muscles after performing third molar extraction.

Диффузная оссификация легких является редкой патологией, которая характеризуется диффузными отложениями мелких фрагментов костной ткани в легких. Заболевание чаще протекает без клинических симптомов и обнаруживается случайно при рентгенологическом обследовании. Иногда возникают одышка, кровохарканье, спонтанный пневмоторакс. Течение болезни хроническое, постепенно прогрессирующее. Как правило, формируется рестриктивный тип нарушения легочной вентиляции. Окончательный диагноз требует его гистологической верификации. Выделяют два гистологических типа легочной оссификации-узелковый (nodules) и ветвистый (dendriform). Представлен клинический случай необычного сочетания диффузной оссификации легких и левостороннего хилоторакса. До госпитализации в клинику института у пациента по месту жительства диагностированы компрессионные переломы двух позвонков и диффузная легочная диссеминация. Заподозрен опухолевый процесс-метастазы анонимного рака, проведен курс лучевой терапии. Через полгода дополнительно развился левосторонний хилоторакс. В связи с отсутствием точного диагноза больной был госпитализирован в клинику. Выполнен ряд клинико-функциональных и лабораторных исследований, которые не позволили верифицировать патологию. Было принято решение о проведении видеоторакоскопии с биопсией легкого справа и клипированием ductus thoracicus. Патогистологическое заключение: диффузная легочная оссификация (ветвистый тип). Послеоперационнный период-без особенностей. Больной направлен для последующего лечения и наблюдения у пульмонолога. Заключение. Впервые описан случай сочетания диффузной оссификации легких и хилоторакса. Выполнение видеоторакоскопии с перекрытием лимфатического грудного протока и последующим патогистологичесим исследованием обеспечивает как получение заключительного клинического диагноза, так и достижение лечебного эффекта. Ключевые слова: диффузная оссификация легких, хилоторакс, диагностика.
