Reducing mortality in severe sepsis with the implementation of a core 6-hour bundle: results from the Portuguese community-acquired sepsis study (SACiUCI study)

Abstract: IntroductionTo evaluate the impact of compliance with a core version of the Surviving Sepsis Campaign 6-hour bundle on 28 days mortality.MethodsCohort, multi-centre, prospective study on community-acquired sepsis (CAS).ResultsSeventeen intensive care units (ICU) entered the study. Over a one year period, 4,142 patients were enrolled in the study. Of the 897 (24%) admitted with CAS, 778 (87%) had severe sepsis or septic shock on ICU admission. In the first six hours of hospital admission: (1) 62% had serum lact… Show more

“…De-escalation of antimicrobial therapy is a mainstay of antibiotic stewardship programs and is associated with less resistant microorganisms, fewer side effects, and lower costs [51]. Several retrospective studies have suggested that obtaining cultures prior to antimicrobial therapy is associated with improved outcome [52,53]. Similarly, de-escalation has also been associated with improved survival in several observational studies [54,55].…”

“…They found an even lower adherence to the full bundle (12%). 11 The low compliance to the resuscitation bundle, but also the 'green line' implementation can have multiple reasons including a low level of awareness of the guidelines by the health professional and/or organizational difficulties hindering their correct implementation. A questionnairebased descriptive study in two national hospitals, evaluated sepsis identification and management skills among health workers, and showed a high percentage of correct answers (particularly by medical doctors) but, nonetheless, a low implementation of the 'green line of sepsis' in the emergency department (EDs).…”
