Recurrent myopericarditis in association with Crohn's disease

Abstract: Myopericarditis is a rare extraintestinal complication of inflammatory bowel disease (IBD). It has also been described as a side-effect of the treatment of IBD. We report a 37-year-old-woman with Crohn's disease who had several mild episodes of myopericarditis, two of which were associated with a pleural effusion, and two with conduction abnormalities in the atrioventricular node. During the last episode, a nodal rhythm was followed by a third-degree atrioventricular block and a prolonged pause, resulting in l… Show more

“…We have presented a pediatric patient with ulcerative colitis who developed cardiac signs and symptoms associated with pericarditis soon after starting a 5-ASA product. Determining the etiology of pericarditis in this setting can be complex as this presentation has been reported as a rare extraintestinal manifestation of inflammatory bowel disease [2][3][4] . Chest pain may be associated with other more common gastrointestinal conditions such as gastroesophageal reflux disease (GERD).…”

“…Severe reactions to mesalamine products are rarely seen in pediatric patients. Cardiac complications have been reported as a rare extraintestinal manifestation of inflammatory bowel disease (IBD) and mainly manifest as pericarditis [2][3][4] . However, cardiac complications may be associated as a very rare drug reaction to 5-aminosalicylic acid (5-ASA) products [1,5,6] .…”

“…54 These symptoms can arise from ulcerative colitis 55,56 or Crohn disease. 57 The cardiac symptoms may be present at the time of initial presentation of the IBD [55][56][57][58][59] or may occur years after diagnosis of either Crohn disease 57 or ulcerative colitis. 60 However, the prevalence of myocarditis, pericarditis, or myopericarditis unrelated to 5-ASA in patients with IBD is very low.…”
