Recombinant human bone morphogenetic protein‐7 in maxillary sinus floor elevation surgery in 3 patients compared to autogenous bone grafts

Abstract: These findings indicate that the OP-1 device has the potential for initiating bone formation in the human maxillary sinus within 6 months after a sinus floor elevation operation. However, the various findings in these 3 patients indicate that the behaviour of the material is at this moment insufficiently predictable, in this indication area. Further investigation is indicated before OP-1 can be successfully used instead of the "gold standard" autogenous bone graft.

“…BMP2 is known to induce osteogenesis and new bone formation 19,20) . BMP7 is a multifunctional member of the BMP family with multiple effects on bone formation and regeneration [21][22][23] . BMP2 and BMP7 homodimers were reported to accelerate bone formation efficiently.…”

“…36,37 A few human trials using BMPs reported to date have utilized superphysiological doses of BMPs that result in "dose-dumping" of potent morphogens in the wound site. 15,38,39 This use of bolus delivery can lead to systemic toxicity, 40 likely through cytokine diffusion into the bloodstream. 41 This approach using ex vivo gene transfer of SDFs provided a more robust response in the stimulation of osteogenesis as compared to our previous efforts using direct gene transfer 42 (and unpublished data).…”

“…An extensive cementogenesis was considered the most significant effect of BMP-7 in bony defects in baboons [122,123]. Improvement of bone formation around titanium implants was also demonstrated by studies in animals [124,125], although no significant benefit was observed when rhBMP-7 was compared with bone grafts for maxillary bony defects in preclinical [126] and pilot human clinical studies [127].…”
