Aim Being subjected to childhood maltreatment has devastating long-term adverse effects and is a major risk factor for mental health problems in adult life. There is empirical support that socio-political factors can be risk factors for childhood maltreatment. Here we examine whether socio-political context predicts self-rated childhood maltreatment in a sample of the German population. Methods Data were drawn from surveys of representative samples of the East and West German population, including participants born 1980 or earlier (5836 participants; 3146 women). Childhood maltreatment was assessed using the Childhood Trauma Screener, the German short screening version of the childhood trauma questionnaire. To examine whether socio-political context is a predictor of childhood maltreatment in women and men, we conducted logistic regression analyses. Results We found that in women, socio-political context (West Germany) significantly predicted childhood maltreatment (odds ratio [95% confidence interval] 1.7 [1.2–2.5], 1.6 [1.1–2.3], 2.5 [1.6–4.1] and 1.8 [1.3–2.5] for emotional neglect, and emotional, physical and sexual violence, respectively). In men, the socio-political context (West Germany) was a predictor of physical and sexual violence (odds ratio [95% confidence interval] 1.8 [1.2–2.7] and 2.5 [1.4–4.5]), respectively. Concerning emotional neglect and violence, socio-political context was not a significant predictor in men. The examination of differential item functioning revealed that our results could not be attributed to differential response behaviour between East and West Germans caused by item bias. Conclusions The results suggest that socio-political context is an important determinant to consider when examining childhood maltreatment. Future research should continue to focus on risk and protective factors at the societal level, such as legal frameworks addressing gender equality and child protection laws, to create further evidence for population-wide prevention strategies ensuring safe, nurturing and thriving environments for children and families.

Purpose Mental distress has become a major public health concern. Temporal trends in psychological distress are complex and depend on numerous factors. In this study, we examined age-period-cohort effects for mental distress including gender and German region over a 15 years’ time span. Methods Data on mental distress from ten cross-sectional surveys of the general German population, covering the years from 2006 to 2021, was used. Hierarchical age-period-cohort analyses including gender and German region as predictors were performed to disentangle age, period, and cohort effects. The Patient Health Questionnaire-4 was used as a brief screener for mental distress. Results We found significant period and cohort effects, with peek values for mental distress in the years 2017 and 2020 and for the oldest birth cohort (born before 1946). Age did not affect mental distress when cohort- and period effects as well as gender and German region were considered. An interaction effect for gender and the German region was found. Women in West Germany reported significantly higher mental distress compared to women in East Germany. Compared to men, women reported the highest prevalence in both regions. Conclusion Important political events as well as major crises can lead to an increase of mental distress in societies. Furthermore, an association between birth cohort and mental distress could be linked to socialization effects of that certain time, causing traumatic experiences or a specific coping style within this cohort group. Prevention and intervention strategies could benefit from acknowledging structural differences linked to period and cohort effects.

Zusammenfassung Ziel der Studie Die Auswirkungen innerdeutscher Migration auf somatoforme Beschwerden (GBB-8) sowie psychologischen Disstress (PHQ-4; erfasst Angst- und Depressionssymptomatik) wurden untersucht. Methodik Eine repräsentative Stichprobe aus dem Jahr 2019 wird herangezogen und in vier Gruppen unterteilt (West, Ost, Ost-West, West-Ost). Es wurde eine Varianzanalyse durchgeführt. Ergebnis Die Gruppe West berichtet signifikant mehr Disstress und somatoforme Beschwerden als die Gruppe Ost. Die Gruppe Ost-West berichten signifikant mehr Disstress und somatoforme Beschwerden. Schlussfolgerung Der Einfluss der innerdeutschen Migration, insbesondere von Ost nach West, sollte in zukünftigen Untersuchungen berücksichtigt werden.
