Введение. Настоящее исследование направлено на изучение прооксидантного потенциала доксорубицин-циклофосфамидного режима химиотерапии наряду с оценкой возможностей антирадикальной защиты триметазидина.Цель. Оценить прорадикальное воздействие АС-режима химиотерапии на миокард наряду с обоснованием применения триметазидина.Материалы и методы. С целью проведения настоящей экспериментальной работы 80 половозрелых инбредных крыс линии Wistar были рандомным образом разделены на четыре равные группы. Модель индуцированного СПОЛ-ассоциированного повреждения миокарда была получена при одновременном применении доксорубицина и циклофосфамида. В качестве потенциального стабилизатора редокс-гомеостаза был использован триметазидин.Результаты. Определены и оценены маркеры окислительно-восстановительного баланса. Полученные данные подтвердили эффективность применения триметазидина как модификатора СПОЛ-индуцированного повреждения сердца и сосудов развившегося на фоне АС-режима химиотерапии.Выводы: 1. АС-режим химиотерапии оказывает СПОЛ-стимулирующее и антиоксидант-лимитирующее воздействие. 2. Триметазидин обеспечивает статистически достоверную нормализацию окислительно-восстановительного гомеостаза миокарда. Introduction. Our research was determined to investigate the Adriamycin + Cyclophos- phamide (AC)-mode of chemotherapy-related disturbances of cardiovascular (CV) ho- meostasis, and possibility of trimetazidine (TMZ)-induced cardioprotection.Purpose. Assess oxidative state of myocardium during AC-mode of chemotherapy ad- ministration, and justification of trimetazidine prescription among under-treatment and post-treatment cancer patient.Materials and methods. For this study, 80 inbred Wistar rats were randomly divided into four equal experimental groups. A model of lipid peroxidation (LPO)-induced myocardial injury, was attained via doxorubicin and cyclophosphamide co-administration. Trimetazi- dine was used as a probable limiter of heart and vessels morpho-functional homeostasis aggravations.Results. The markers of redox-balance, were determined and assessed. The data obtained showed that this TMZ modified the intensification of lipid peroxidation.Conclusions: 1. AC-mode of chemotherapy has LPO-stimulating and antioxidant-limiting impact. 2. TMZ provide statistically significant and valuable normalization of redox-ho- meostasis.

Personalized trimetazidine prescription as a cytoprotective agent in patients with coronary artery disease

Aim. To develop a personalized approach to the trimetazidine use in patients with coronary artery disease (CAD) based on the criteria for predicting the cytoprotective activity tested in vitro.Material and methods. We examined 30 patients with class I-III stable effort angina with concomitant hypertension and heart failure. The patients underwent echocardiography, complete blood count, biochemical tests with determination of the lipid profile, creatine phosphokinase (CPK), CPK-MB, renal and hepatic parameters. To determine the cytoprotective activity of trimetazidine, white blood cells (WBCs) of patients were examined in vitro using an Eclipse Ti-U inverted fluorescence microscope (Nikon, Japan). Living and dead cells were determined by staining WBCs with fluorescent dyes (Calcein AM, Ethidium bromide). Cell viability index (CVI) was calculated. The statistical processing was carried out. The criteria for predicting the trimetazidine cytoprotective effect were determined using Wald statistics.Results. When trimetazidine was injected into a WBC suspension sample, two types of cell viability changes were observed: in 60% of patients, CVI increased, on average, by 37% (from 23% to 60%, p<0,001) and in 40% of patients, CVI decreased, on average, by 30% (from 54% to 24%, p<0,05).A number of conditions of the patient initial status were identified for the manifestation of trimetazidine cytoprotective activity: grade 1 hypertension; right ventricular end diastolic dimension up to 30 mm according to echocardiography; normal lipid profile with a total cholesterol <5,3 mmol/L, very-low-density lipoproteins <1 mmol/L and an atherogenic coefficient up to 3 CU, myocyte and cardiomyocyte destruction (total CPK >100 U/L and CPK-MB >15 U/L), normal liver function (alanine aminotransferase <25 U/L), renal dysfunction (total protein <75 g/L, urea >8 mmol/L and blood creatinine >100 pmol/L), normal thrombopoiesis (immature platelet fraction <5%) and the state of functional adaptive system resistance (blood lymphocytes <30% and neutrophils >4x109/L).Conclusion. According to this in vitro analysis, the trimetazidine significantly increases (by an average of 37%) the cell (WBC) viability in 60% of patients with CAD. There are conditions of patient initial status, which specifies an individual pharmacodynamic target for the cytoprotective action of the drug.

The objective: to assess the impact of tuberculosis (TB), chronic obstructive pulmonary disease (COPD) and chronic heart failure (CHF) on the quality of life (QOL) and the need to manage each disease to improve the physical and mental components of QOL.Subjects and Methods. A simple prospective study included 80 patients, men – 53.9%, women – 46.1% who were divided into 3 groups: Group 1 – COPD (n = 35), median age [Me (25.0; 75.0 )] 56.0 (54.0; 60.0) years old, Group 2 – COPD + TB (n = 25), 58.0 (56.0; 59.0) years old, Group 3 – COPD + TB + CHF with preserved ejection fraction (n = 20), 60.0 (57.0; 64.0) years old. QOL was assessed using the SF-36 questionnaire. There were no statistically signifcant diﬀerences between the compared groups in terms of COPD severity and TB forms.Results. Low QOL indices were observed in TB + COPD + CHF Group assessed by all scores especially for physical (30 points) and social functioning (35 points) and general health (35 points). After 3 months of therapy aimed to manage all pathological conditions, a positive trend in QOL was achieved in TB + COPD + CHF Group in terms of physical and mental health scores.Conclusion. QOL in co-morbid patients is determined not only by the quality and severity of individual nosologies but also by their number. Comprehensive therapy improves the general subjective state of the patient that helps to improve adherence to drug therapy thereby increasing the chance of recovery.
