Background: Preemptive analgesia has as its basic principle the administration of analgesics before the onset of painful stimuli, in order to reduce or prevent postoperative pain, but this question is little explored in implantology. Thus, this study was conducted in order to evaluate the clinical efficacy of ibuprofen in pain prevention after unit implant surgery. Material and Methods: For this triple-blind, parallel, placebo-controlled and randomized clinical trial, 54 insertion surgeries of unitary implants were performed. Two groups have received two different protocols 1 hour before surgery: Ibuprofen group (IBU) 600 mg of ibuprofen; and (2) placebo group (maize starch). The intensity of the pain was evaluated through the visual analogue scale (VAS) in 6 times (1, 6, 12, 24, 48 and 72 hours after the surgery). Patients were instructed to take 750 mg of paracetamol as rescue medication, if necessary. The occurrence and the intensity of pain were analyzed by means of an analysis of variance ANOVA with repeated measurements using the general linear model procedure. Results: The IBU group had significantly lower VAS scores overall (IBU = 0.30, ± 0.57; placebo = 1.14, ± 1.07; p<0.001) and at all times in the intra, intergroup comparisons and time/group interaction than the placebo group (p< 0.001). The use of rescue medication was significantly lower and the postoperative time was longer in the IBU group compared to placebo (p = 0.002). Conclusions: The single use of ibuprofen was found to be significantly superior in reducing pain after unit implant surgery compared to placebo.

The history of evidence-based use of non-steroidal anti-inflammatory drugs (NSAIDs) goes back at least two hundred and fifty years. Over the past period, the path has been passed from the use of willow bark decoctions to the synthesis and introduction of selective cyclooxygenase inhibitors into clinical practice. To date, the research direction has shifted from the search for new substances to the search for new routes of administration. The wide range of existing drug substances, however, has only in a few dosage forms. Thus, ketorolac tromethamine is known only as a solution for parenteral and intranasal administration, as well as oral tablets. This drug belongs to NSAIDs, particularly non-selective cyclooxygenase inhibitors, and shows a pronounced analgesic activity. Due to this property, ketorolac can serve as an alternative to opioid analgesics or can reduce the dosage of the latter when used in combination. However, a number of systemic side effects (ulcerogenic properties, negative effect on the blood), unfortunately, impose their limitations. A possible solution to this situation may be the creation of local delivery systems, in particular, in situ implants. This review highlights the problem of developing local systems for the delivery of ketorolac tromethamine for the relief of acute pain. Special attention is paid to in situ implants based on bioadhesive polymers.

This article aims to associate patient-reported pain intensity in the immediate seven days after dental implant surgery with surgical factors, sex and patient age. The sample was composed of 108 patients from a dental school in southern Brazilian, between 2018 and 2020. The variables torque, pre- and postoperative medication, healing of first and second intention, gender, age, number of quadrants, number of implants and type of surgery were related to the outcome pain. Pain was reported every day by the patient until the seventh day after surgery, using a visual analogic scale (VAS). Univariate Poisson regression models were used to assess the relationship among pain and the factors. Rate ratios were obtained with 95% of confidence intervals. Overall pain was reported as moderate/intense (VAS: 3-10) by 30.56% of patients, mild pain (VAS: 1-3) by 55.56%, and no pain symptoms were reported by 13.89% of patients. Individuals which the implants were installed with high torques (50-80 N) showed more pain (p=0.03) compared to patients which the implants were installed with regular torque (30-45 N). The other factors evaluated were not significant. The torque was the most relevant factor related to pain among the evaluated by the study. High torque generates more postoperative pain than lower torque. The factors gender, age, number of operated quadrants, number of installed implants, type of surgery and pre and postoperative analgesic did not interfere in the patient's pain during the first week after surgery.
