Pulmonary hydatid cysts: the naturally occurring models for rupture

Abstract: At the fistula site, the intracystic pressure is unopposed, leading to herniation of the endocyst membrane, disruption of its integrity, and rupture. Therefore, we postulate that this scenario in combination with other coexisting factors could be the possible mechanism for cyst rupture in group 1. This concept may also explain the pathogenesis of the high rate of rupture of pulmonary hydatid cysts. Accordingly, we consider these cysts a naturally occurring model for rupture, which should be treated surgically … Show more

“…In our study, more than half of the cases involved perforated cysts (50.4%). A very similar frequency (53.1%) was seen in a study conducted by Ashour et al [9], but a lower rate (41%) was observed in children under 16 years old in a study conducted by Onal and Demir [10]. The male-to-female ratio in the present study was 1:1.5 for group 1 and 1:1.7 for group 2, while it was 2:1 in both groups in the study by Ashour et al [9].…”

“…This increased tension on the wall of a cyst, resulting in its rupture, has been proven elsewhere as a cause of rupture of pulmonary hydatid cyst. 17 In a retrospective study conducted in China on 337 patients with PSP, a strong correlation was found between the occurrence of PSP and fall in the mean atmospheric pressure. The mean atmospheric pressure in PSP days was obviously lower than that of non-PSP days.…”

“…Clinical presentation depends on whether the cyst is intact or ruptured [4,11]. Rupture of the cyst into bronchus causes hemoptysis or expectoration of the cystic content [12][13][14][15]. Purulent sputum with fever might occur in cases with infected cyst.…”

“…In addition to these, sometimes life-threatening complications might arise following the rupture of HC. Rupture of cyst may lead to asphyxia due to flooding of bronchial tree with hydatid fluid or blockage of bronchial lumen due to germinal membrane [12]. We had two cases where emergency intervention and surgery were needed.…”
