Background and Objectives:To evaluate the association between psoriasis and migraine. Patients and Methods: MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials were searched for relevant observational studies from their respective inception to May 1, 2022. A random-effects model meta-analysis was performed to calculate the risk estimates quantifying the associations between psoriasis and migraine. We also performed a sensitivity analysis by including only studies with adjusted risk estimates and a subgroup analysis according to the severity of psoriasis. Results: We included 9 studies with 6,742,075 participants. The meta-analysis illustrated increased odds for prevalent migraine among patients with psoriasis (pooled OR: 1.69, 95% CI: 1.26-2.28) and increased odds for prevalent psoriasis among those with migraine (OR: 1.88, 95% CI: 1.32-3.67). A subgroup analysis of cohort studies demonstrated an increasingly higher risk of incident migraine in patients with mild psoriasis and severe psoriasis (IRR being 1.37 (95% CI 1.30-1.44) and 1.55 (95% CI 1.29-1.86), respectively). Conclusions: This meta-analysis revealed significant bidirectional associations between migraine and psoriasis. Greater severity of psoriasis appears to be associated with a higher risk of developing migraine. Clinicians should evaluate symptoms of migraine in patients with psoriasis and provide proper treatments.

ZusammenfassungHintergrund und ZieleDie Assoziation zwischen Psoriasis und Migräne zu beurteilen.Patienten und MethodikMEDLINE, Embase und Cochrane Central Register of Controlled Trials wurden vom Zeitpunkt ihrer jeweiligen Gründung bis zum 1. Mai 2022 nach relevanten Beobachtungsstudien durchsucht. Die Risikoeinschätzung zur Quantifizierung der Assoziationen zwischen Psoriasis und Migräne wurde anhand einer Metaanalyse mit Zufallseffektmodell berechnet. Außerdem führten wir eine Sensitivitätsanalyse durch ausschließliche Berücksichtigung von Studien mit bereinigten Risikoschätzwerten sowie eine Subgruppenanalyse gemäß der Schwere der Psoriasis durch.ErgebnisseEs wurden 9 Studien mit 6 742 075 Teilnehmern eingeschlossen. Die Metaanalyse verdeutlichte die erhöhte Prävalenz der Migräne bei Patienten mit Psoriasis (gepoolte OR: 1,69, 95%‐KI: 1,26–2,28) sowie die erhöhte Prävalenz der Psoriasis bei Patienten mit Migräne (OR: 1,88, 95%‐KI: 1,32–3,67). Eine Subgruppenanalyse der Kohortenstudien ergab ein zunehmend höheres Risiko einer inzidenten Migräne bei Patienten mit leichter und schwerer Psoriasis (die IRR betrug 1,37 (95%‐KI 1,30–1,44) beziehungsweise 1,55 (95%‐KI 1,29–1,86)).SchlussfolgerungenDiese Metaanalyse ergab signifikante bidirektionale Assoziationen zwischen Migräne und Psoriasis. Ein höherer Schweregrad der Psoriasis ist anscheinend mit einem höheren Risiko assoziiert, eine Migräne zu entwickeln. Mediziner sollten die Symptome einer Migräne bei Patienten mit Psoriasis abklären und eine geeignete Behandlung anbieten.

Background Although many studies have demonstrated the commodity of psoriasis with migraine and indicated that they may have similar susceptibility genes and pathophysiologic mechanism, the clinic association between the migraine and psoriasis remains unclear. Methods We have already searched Pubmed, Embase, and Web of Science for case–control, cross-sectional, or cohort studies, and extract rate, odds or risk of migraine in subjects with psoriasis or without psoriasis. Using defined inclusion and exclusion criteria, finally include nine studies. Pooling of the suitable data was applied when necessary. Results Five cross-sectional studies included 6355 psoriasis patients and 934413 controls, migraine highly occurred in psoriasis patient (pooled OR 1.64; 95% confidence interval [1.28; 2.11]). In addition, with 4375 psoriasis patients provided, the rate of migraine occurred in psoriasis patient (pooled rate 0.21; 95% confidence interval [0.13; 0.35]). Conclusion Migraine and psoriasis present a clear co-occurrence and similar pathophysiologic mechanism, which lead to the assumption that the two diseases might be linked. Screening and selection of proper assessment of migraine among psoriasis patients are warranted and needed.
