Breast milk is recommended for very preterm infants but fortification is required to increase its nutrient density in order to promote growth and development. Even with fortification, those born extremely preterm and those who are fluid restricted may not achieve intrauterine growth targets. Thus, fortification beyond routine amounts may be necessary for some infants. Further study is necessary to determine optimal methods, types and amounts of fortification, as well as upper limits of osmolality, so as to ensure avoidance of feeding intolerance and necrotizing enterocolitis whilst achieving appropriate rate of weight gain and accretion of nutrients. The efficacy of new formulations of fortifiers and infant formulae needs further study. Key points• Breast milk requires fortification to meet the nutrient needs of preterm infants; monitoring protein intake via blood urea nitrogen to guide fortification looks promising.• Breast milk fortifiers have variable effects on osmolality of fortified expressed breast milk.• Randomised controlled trials are required to assess the efficacy of recently revised formulations of breast milk fortifier and preterm formulae in the context of their ability to meet nutritional requirements for growth and effect on osmolality and feeding intolerance.• Use of gum-based thickening agents for treatment of regurgitation is associated with increased risk of necrotising enterocolitis in preterm and term infants.

“…The reported increases in milk production during the first 2 weeks as a result of double pumping may be important in order to build reserves of breastmilk and maternal capacity to continue producing milk in later weeks. Two papers 195,196 have suggested setting a goal for daily milk volume of around 750 ml/day to encourage mothers to continue to express on a regular basis. This consideration further supports the use of double pumping during the first 2 weeks to promote sufficient mother's own milk both to meet the needs of her newborn preterm infant and to establish lactation.…”

Source of data:The authors made an extensive literature review on the topic, including technical books, theses, publications of national and international organizations, and search on MEDLINE database (1990 to 2003), using the following key words and boolean operators: breastfeeding AND low birth weight and breastfeeding AND preterm infant. Some significant references cited in the reviewed publications were used as well.Summary of the findings: After this review we conclude that many aspects make the breast milk particularly suitable to the premature newborn feeding. Despite being highly desirable, little success in breastfeeding preterm infants is generally observed, particularly in special care neonatal units, although there are evidences suggesting that a highly supportive hospital environment can make it possible to breastfeed these infants.Conclusions: Although breastfeeding premature infants represents a challenge, it is feasible if appropriate help and support are provided.
