Breastfeeding is the natural and safe way of feeding small infants, providing nutritional, immunological, psychological and economic recognized and unquestionable advantages. These qualities are especially important in premature infants, because of their vulnerability. Despite highly desirable, there is, in general, little success in breastfeeding preterm infants, especially in special care neonatal units. There are evidences that a high supportive hospital environment, with an interdisciplinary team, makes possible to these infants to be breastfed. In this article, the authors present an up-to-date review about the components of human milk and its unique characteristics, as well as describes aspects that make the breast milk particularly suitable for feeding the premature newborn. Objective: To describe breastfeeding prevalence and associated factors, in the municipality of Londrina, PR, Brazil. Methods: This quantitative, descriptive study included interviews of 770 companions of children younger than 12 months of age during an anti-polio vaccination campaign in August 2008. Results: A total of 72.5% of the children were breastfed in the first hour after birth; 33.8% were exclusively breastfed from zero to six months of age, 53.7% until the fourth month and 7.8% until the sixth month; 51.5% were in continued breastfeeding between 9 and 12 months of age. The women who breastfed the most were 35 years old or older, had a bachelor's degree, had more than one child and were on maternity leave. Conclusion: Breastfeeding rates increased in the city, however, analyzing health services actions is needed to improve these rates. ReSumo Objetivo: Descrever a prevalência do aleitamento materno e fatores associados no município de Londrina-PR. Métodos: Estudo quantitativo, descritivo, entrevistando 770 acompanhantes de crianças menores de 12 meses, durante a campanha de vacinação antipoliomielite, em agosto de 2008. Resultados: Na primeira hora, foram amamentadas 72,5% das crianças; 33,8% estavam em aleitamento materno exclusivo de zero a seis meses, no quarto mês, 53,7% e no sexto mês 7,8%; 51,5% em aleitamento materno continuado entre 9 e 12 meses. OBJETIVOS: identificar a prevalência do aleitamento materno exclusivo entre lactentes atendidos na rede pública municipal e sua associação com variáveis maternas e do lactente. MÉTODOS: estudo transversal realizado durante a Campanha Nacional de Vacinação em 2005, em Joinville, Santa Catarina, Brasil. Foi aplicado um questionário aos acompanhantes de 889 lactentes menores de um ano de idade nos postos de vacinação incluindo aspectos maternos e do lactente. Para a análise, utilizou-se estatística descritiva e o teste do qui-quadrado, com nível de significância de 5%. RESULTADOS as prevalências de amamentação foram 90,7%, 84,2% e 73,5%, nos lactentes com idade inferior a quatro, seis e doze meses, respectivamente. O índice de aleitamento materno exclusivo foi de 53,9% em menores de quatro e de 43,6% em menores de seis meses. O uso de chupeta esteve significativamente associado à amamentação não-exclusiva nas crianças com menos de quatro ou seis meses de idade (p<0,001) e o trabalho materno, entre aquelas com até seis meses (p=0,001). CONCLUSÕES: as taxas de amamentação exclusiva entre menores de um ano de idade atendidos na rede pública estão aquém do preconizado pela Organização Mundial da Saúde. O uso de chupeta e o trabalho materno podem interferir negativamente com o aleitamento materno exclusivo.

Breastfeeding is the natural way of feeding infants, providing nutritional, immunological, psychological, and economic advantages. These qualities are especially important in premature neonates because of their vulnerability. Nevertheless, a lower incidence of successful breastfeeding has been observed in this population. This prospective study investigated the frequency of mother's milk use on discharge of premature neonates from an at-risk neonatal unit. It involved 244 premature neonates and took place at a Brazilian Baby-Friendly Hospital. The observed frequency of mother's milk use at neonatal unit discharge was 94.6%. The finding that most premature neonates were receiving mother's milk reflects a well-structured program for promoting breastfeeding of premature neonates at this institution, which relies on an interdisciplinary team to provide adequate support to the mothers.
Estudo exploratório para verificar a utilização da colostroterapia em recém-nascidos de muito baixo peso, em unidade neonatal de maternidade de médio porte, por meio da avaliação de prontuários. Dos 108 prontuários avaliados, 56,5% dos neonatos eram do sexo feminino, com média de peso e idade gestacional ao nascimento de 1091,9 g (DP=263,7g) e 29 semanas/2dias (DP=2semanas/6dias), respectivamente. A colostroterapia foi utilizada em 25,9% dos pacientes, sendo que em 96,4% dos casos a administração foi por via oral. O uso de terapia colostral foi associado significativamente a variáveis neonatais como idade gestacional, peso ao nascer e óbito neonatal (p = 0,001, p < 0,001, e p < 0,001). Não havendo protocolo estabelecido para guiar a prescrição da colostroterapia, esta ocorreu principalmente para prematuros de menor peso, mais imaturos ou mais doentes, com maior risco de morbidade e mortalidade.
