ГБУЗ МО «Московский научно-исследовательский институт акушерства и гинекологии» (дир. -акад. РАН, проф. В.И. Краснопольский) Минздрава Московской области, Москва, Россия Представлены результаты выполненных в последние годы научных исследований, посвященных принципам ультразвуко-вой диагностики состояния плода при сахарном диабете у беременной. Подробно изложены ультразвуковые признаки макросомии плода, выделены два ее типа: симметричный и асимметричный. Подчеркнуто, что раннее выявление асим-метричности формы макросомии позволяет диагностировать скрытые формы гестационного сахарного диабета. Изло-жены размеры плода (диаметр живота, окружность живота, окружность головки, бипариетальный размер головки, диа-метр груди, толщина подкожной клетчатки, диаметр «щека-к-щеке») и их соотношения (пондераловый индекс, буккальный индекс и др.), позволяющие уточнить состояние плода при сахарном диабете у беременной. Указаны патогномоничные эхографические признаки, характерные для внутренних органов плода (поджелудочная железа, сердце, легкие, печень и др.), плацентарной недостаточности, функциональной зрелости, гемодинамики плода при диабетической фетопатии.Авторы информируют об отсутствии конфликта интересов. The review gives the results of recent researches dealing with the principles of fetal ultrasound diagnosis in a diabetic pregnant woman. The ultrasound signs of fetal macrosomia are detailed and its 2 types (symmetric and asymmetric) are identified. It is emphasized that the early detection of asymmetric macrosomia allows the diagnosis of occult gestational diabetes. The authors set forth fetal dimensions (abdominal diameter, abdominal circumference, head circumference, biparietal head size, chest diameter, subcutaneous fat thickness, and cheek-to-cheek diameters) and their ratios (ponderal index, buccal index, and others), which can clarify the fetal status in diabetic pregnant women. Pathognomic echographic signs that are characteristic of fetal visceral organs (pancreas, heart, lung, liver etc.), placental insufficiency, functional maturity, and fetal hemodynamics in diabetic fetopathy are indicated.The authors declare no conflicts of interest.

Gestational diabetes mellitus (GDM) is a disease, which is characterized by glucose intolerance first recognized during pregnancy, but which does not meet criteria of overt diabetes. GDM is a major medico-social problem, because is associated with adverse pregnancy outcomes. The main method of GDM treatment is nutrition therapy, which can be supplemented with anti-hyperglycemic agents. Lifestyle modification has shown to be effective in preventing the complications of GDM. Despite this fact, there is no consensus on which dietary recommendations should be recommended in case of glucose intolerance during pregnancy. Most of current guideline recommendations suggest to control carbohydrate or calorie intake, diet with low glycemic index, Mediterranean or DASH diet. Another essential aspect of nutrition therapy in GDM is a safety of a diet outcomes for both mothers and their offspring. The purpose of this review is to evaluate current recommendations for GDM nutrition management and their supporting evidence. Along with this the goal is to the review common and uncommon proposed GDM diets, and ultimately and to provide an analysis of their safety and efficacy, the possibility of application in clinical practice for the treatment of patients with GDM.
