Primary pulmonary choriocarcinoma combined with adenocarcinoma

Abstract: An extremely rare autopsy case of primary pulmonary choriocarcinoma combined with adenocarcinomatous components in a 77-year-old Japanese man is described. The patient died of rapidly progressive respiratory dysfunction without ante-mortem diagnosis. Autopsy revealed necro-hemorrhagic areas of the primary lung tumor with a typical biphasic pattern of choriocarcinoma. Topographical analysis suggested that moderately to poorly differentiated adenocarcinoma components partially surrounded the choriocarcinomatous … Show more

“…Although one may question the hepatic tumor in these three cases that represent metastasis, the presence of a solitary tumor in the liver and multiple small nodules in the lung, adrenal glands, and peritoneum were more consistent with a primary hepatic choriocarcinoma with metastasis to the latter sites rather than the opposite. In addition, some carcinomas such as rectal adenocarcinoma [31], gastric adenocarcinoma [32], and lung adenocarcinoma [33] can have choriocarcinomatous differentiation. Theoretically, the choriocarcinomatous component can metastasize to the liver.…”

“…3 Primary pulmonary location of germ cell tumor is extremely rare with less than 40 cases of choriocarcinomas and five cases of yolk sac tumors 1,4-7 have been reported thus far. A smaller number of cases have been reported as a mixture of cho- riocarcinoma with adenocarcinoma, [8][9][10] large cell carcinoma, 11,12 or small cell carcinoma. 13 In a survey performed in 2004, Umemori and his colleagues reviewed 25 previously described cases of primary pulmonary choriocarcinoma (PPC) to establish the clinical characteristics and most appropriate management of this disease.…”

“…12 A review of the English medical literature, the terms "Pulmonary collision tumor" found cases of primary tumor of lung with another tumor like chronic lymphocytic leukemia, 13 colon, 14 choriocarcinoma, 15 non-Hodgkin lymphoma, 16 malignant melanoma, 17 metastatic adenoid cystic carcinoma, 18 breast, 19 large cell neuroendocrine carcinoma, 20 and as documented above with large cell carcinoma. 7 A PubMed search using the term pulmonary collision and carcinoid found only 3 cases with marginal zone lymphoma, 21 high-grade spindle cell sarcoma, 22 and mesothelioma.…”
