Introduction: Urinary tract infections (UTI) are one of the most common infections encountered in clinical settings, being amenable to easy diagnosis with established laboratory methods. However, in recent years, antibiotic resistance has become a major public health problem worldwide owing to the extensive and inappropriate use of antimicrobial agents. Objectives: 1. To determine the various Gram positive isolates causing urinary tract infections. 2. To determine the antibiotic susceptibility pattern of Gram positive cocci. Material and Methods: A cross sectional study was conducted at a private tertiary care hospital in Shivamogga district of Karnataka, using secondary data of Gram positive bacteria isolated from urine samples maintained in the Microbiology laboratory registers for a period of 1 year from January 2016 to December 2016. Standard protocols were followed for bacteria isolation, identification and to assess their antibiotic susceptibility. Statistical analysis was done using MS office Excel 2010. Results: Of the total 1254 urine specimens, 466 (37.16%) revealed positive cultures. Out of the 512 isolates from positive cultures, 196 (38.28%) were Gram positive bacteria. The predominant isolates i.e. 86 (43.87%) belonged to Enterococcus species. All the isolates were susceptible to Linezolid and Vancomycin. Majority i.e. 184 (93.87%) were sensitive to nitrofurantoin followed by gentamicin 146 (74.48%) and majority showed resistance to ampicillin i.e.126 (64.28%). Conclusion:The rise in the resistance among uropathogens alerts us against indiscriminate use of antibiotics, indicating the need to establish antibiotic policies along with stringent measures to ensure effectiveness of the same.

Because of indiscriminate, inadequate and irrational usage of antimicriobials, the sensitivity pattern of community acquired uropathogens is changing drastically, specifically in developing countries, such as Bangladesh. This study was undertaken to investigate the profile of common uropathogens in community settings and to evaluate their antimicrobial resistance profile in order to categorize them into multidrug resistant (MDR), extensively drug resistant (XDR) and pandrug resistant (PDR) organisms.Total 800 urine samples were collected from symptomatic UTI cases and processed in the Department of Microbiology of DMC following standard microbiological methods. Antimicrobial susceptibility pattern was determined by Kirby Bauer Disc diffusion method following Clinical and Laboratory Standards Institute (CLSI) guidelines.Simultaneously, the percentage prevalence of MDR, XDR, and PDR isolates were alsodetermined. Out of the 800 urine samples screened, 150 (18.75%) samples yielded significant growth. The most common bacterial isolate was Esch. coli (60%), followed by Klebsiella species (13.33%). A significantly high resistance was noted to the beta-lactam group of antimicrobials, fluoroquinolones and cotrimoxazole, both by the gram-negative bacilli (GNB) as well as gram-positive cocci (GPC). However nitrofurantoinwas found to be most effective oral drug against both GNB and GPC. Out of total 150 bacterial strains studied, 106 (70.67%) bacterial strains were MDR, 21 (14%) strains were XDR, and no PDR was detected.So, close monitoring of MDR, XDR, or even PDR must be done by all clinical microbiology laboratories to implement effective measures to reduce the menace of antimicrobial resistance.

Background: Extended-spectrum β-lactamase (ESBL)-producing Klebsiella pneumonia has become widespread in hospitals and is increasing in community settings. Most of K. pneumonia that harbor these enzymes, display resistance to other unrelated antimicrobial agents and thus, often pose a therapeutic dilemma. Objectives: This study was conducted to determine the prevalence of ESBL-producing K. pneumonia in a major university hospital in Zahedan, Iran. Materials and Methods:The susceptibility of 83 K. pneumonia isolates to 12 antibiotics was assessed using Kirby-Bauer disc diffusion method. For the ESBL phenotypic test, double-disk diffusion (DD) method was used. Results: The highest resistance rates of the isolates were seen against cefixime (82%), cefotaxime (81%), ceftriaxone (73%), ceftazidime (72%), and azithromycin (60%), consecutively. The lowest resistance rates were observed against gentamicin (58%), tetracycline (59%), nalidixic acid (59%), and amikacin (63%), consecutively. ESBLs were found in 65% of K. pneumonia isolates. Conclusions: We found that 65% of K. pneumonia isolates produced ESBL. Therapeutic strategies to control infections should be carefully formulated in teaching hospitals. The high percentage of drug resistance in ESBL-producing K. pneumonia suggests that routine detection of ESBL by reliable laboratory methods is required.
