Objective: to understand the early sexual initiation process among male adolescents attending school in the context of gender diversity, in the light of Madeleine Leininger's Theory. Method: a descriptive and exploratory research study with a qualitative approach, conducted with 18 adolescents attending school in the city of Recife-Pernambuco, Brazil. The individual interviews were conducted from June to December 2018 and a semi-structured interview script was used to produce the empirical material. As analytical resource, the IRAMuTeQ® software was resorted to in order to categorize the data in the thematic modality. Results: four thematic axes emerged from the interviews, namely: Influencing factors in early sexual initiation; Experience in early sexual initiation; Gender identity in the context of multiple masculinities; and Cultural aspects related to early sexual initiation. Sexual initiation emerges as a requirement for male adolescents to be respected and accepted in a group. It is understood that early sexual initiation is mainly characterized by emotional immaturity and by lack of knowledge, exposing adolescents to situations of vulnerabilities. Conclusion: taboos inhibit the establishment of dialogical relations in the family context and in the school setting, configuring situations of vulnerability for early sexual initiation, given the findings of sexuality in the context of gender diversity. Interdisciplinary and intersectoral actions, with emphasis on nurses' articulating role, become essential to foster dialogical spaces of congruent care for adolescents aiming at constructing knowledge about gender diversity and safe sexual initiation.

OBJECTIVE: This study aimed to assess live birth frequency and age-specific fertility rates (ASFR) in the period 1996-2018 and the number of pregnancies at <14 years old in the period 2012-2018.METHODS: This was a cross-sectional study conducted by search on Live Births Data System (SINASC/DATASUS) database.RESULTS: There was a variation in ASFR in Brazil of 0.78‰ in 1996 to 0.87‰ in 2018 (+11.5%). In the north region, it increased from 1.28‰ to 1.66‰ in 2018. In the northeast region, it increased from 0.72‰ to 1.66‰ (+131%) in 1996-2011, but decreased to 1.31‰ in 2018 (-21% in relation to 2011). When comparing 1996 and 2018, in the southeast region, there was a 22% decrease; in the south region, it was 48.2%; and in the Center-West region, it was 34%; but in the north region, there was a 29.7% increase, and in the northeast region, it was 81.9%. When adding girls who became pregnant aged 13 years and gave birth at 14, there was a threefold increase in the rate.CONCLUSIONS: The increase of pregnancies in <14 years old in less developed regions of Brazil shows an association with socioeconomic factors and reveals the severe problem of rape of vulnerable persons in the country.

Este estudo foi realizado com o objetivo de identificar os fatores de vulnerabilidades em que as crianças são violadas em sua infância por conta das violências sexuais. Trata-se de uma revisão integrativa, utilizando os seguintes descritores: violência sexual, criança, infantil, infância, vulnerabilidade, nas línguas portuguesa, inglesa e espanhola. Os dados obtidos permitiram a categorização e análise das vulnerabilidades individuais e sociais abordadas nos 80 textos científicos analisados. A violência sexual infantil acarreta consequências negativas para o desenvolvimento neurológico, emocional, físico e social das vítimas, sendo responsabilidade de todos o enfrentamento efetivo a essa violação. A bioética, na tentativa de garantir o respeito à dignidade e a proteção à infância, busca desvelar e promover ações de reflexão e de prevenção da violência contra essa população vulnerável.
