SUMMARY INTRODUCTION Teenage pregnancy is a universal phenomenon, with higher prevalence in developing countries. Although there has been a reduction in Brasil since the year 2000, the age-specific fertility rate for this age group remains high. OBJECTIVE To evaluate the frequency of adolescence pregnancy in in Brasil from 2006 to 2015 and its association with the Human Development Index (HDI). METHODS A descriptive epidemiological study, conducted by searching the database of the Department of Informatics of the Unified Health System (DATASUS), using information from the Information System on Live Births (SINASC) for the five Brazilian regions. RESULTS There was a reduction in the percentage of live births (LB) from adolescent mothers (10 to 19 years old) in Brasil by 13.0% over the last ten years. This decline was observed in all Brazilian regions among mothers aged 15 to 19 years. The number of LB increased by 5.0% among mothers aged 10 to 14 years in the North and decreased in the other regions, with higher rates in the South (18.0%). The specific fertility rate for the 15-19-year-old group decreased from 70.9/1,000 to 61.8/1,000 in the period. The proportion of LB is inversely associated with the HDI, except in the Northeast (the lowest HDI in the country), where there was a significant reduction (18.0%) among mothers aged 15-19 and 2% among those aged 10-14 years. CONCLUSION Teenage pregnancy in Brasil is in slow decline, especially among mothers aged 10-14 years and is inversely associated with the HDI, except in the Northeast.
This study aimed to establish the safety of chemotherapy use in pregnant women with breast cancer, and to find possible effects in the fetus. A search of MEDLINE/PubMed, LILACS, SciELO, Cochrane, UpToDate, and Google Scholar databases was performed to identify publications, 86 articles published from 2001 to 2012 were retrieved and evaluated by two readers in accordance predetermined exclusion and inclusion criteria; 39 articles were selected. All the chemotherapy drugs used to treat breast cancer during pregnancy belonged to class D, and consisted of 5-fluorouracil (F), doxorubicin (A) or epirubicin (E) and cyclophosphamide (C), or the combination doxorubicin and cyclophosphamide (AC), a safe regimen when used after the first trimester of pregnancy. Few studies evaluated the use of taxanes (T), such as docetaxel (D) and paclitaxel (P), with no increase in the occurrence of fetal defects and other maternal complications when used in the second and third trimesters of pregnancy. The use of trastuzumab in pregnant women is associated with oligohydramnios and anhydramnios; thus, it is not recommended during pregnancy. As almost all studies were observational and retrospective, new prospective studies on the subject are needed.
ResumoDevido ao aumento da idade materna e à epidemia de obesidade, a prevalência de gestantes com diabetes tipo 2 ou que desenvolvem diabetes mellitus gestacional (DMG) tem sido cada vez maior. A associação do diabetes com aumento do risco de complicações materno-fetais é conhecida há muitos anos, mas foi apenas em 2008 que a forte e contínua relação entre níveis glicêmicos acima do normal e risco de complicações fetais foi confirmada por meio de um grande ensaio clínico prospectivo. Atualmente, a investigação quanto à presença de diabetes na gestação é recomendada para todas as gestantes, pela realização de glicemia de jejum na primeira consulta do pré-natal e de TOTG (Teste Oral de Tolerância à Glicose) entre a 24ª e a 28ª semanas. O tratamento realizado por equipe multidisciplinar e com objetivo de manter o controle glicêmico o mais próximo possível do normal possibilita a redução das taxas de morbimortalidade materno-fetais. O objetivo desta revisão é discorrer sobre epidemiologia, fisiopatologia, diagnóstico, complicações, acompanhamento e tratamento de gestantes portadoras de diabetes tipo 1 ou 2 e das portadoras de DMG, ressaltando-se as semelhanças e peculiaridades de cada caso, com base nos principais estudos e consensos mais recentes.Descritores: Diabetes mellitus; Diabetes gestacional; Gestação. AbstractDue to maternal aging and the obesity epidemic, the prevalence of type 2 diabetes mellitus among pregnant women and gestational diabetes mellitus is increasing. The association of diabetes and increasing risk of maternal and fetal complications has been known for several years, but only in 2008 the strong and continuous relationship between high glucose levels and the risk of fetal complications was confirmed in a large prospective clinical trial. Currently, diabetes screening is warranted in every pregnant woman through the measurement of fasting plasma glucose in the first pre-natal evaluation and OGTT (Oral Glucose Tolerance Test) between the 24 th and 28 th gestational weeks. Treatment is carried out by a multidisciplinary team, aiming at achieving near-normal glucose levels, leading to the reduction of maternal and fetal morbidity. The purpose of this review article is to discuss the epidemiology, physiopathology, diagnosis, complications, treatment and follow-up of pregnant women with pregestational diabetes and gestational diabetes, pointing out the similarities and peculiarities of each condition based on the most important studies and the most recent consensus.Keywords: Diabetes mellitus; Diabetes, Gestational; Pregnancy. HTLV-1/2 são retrovírus com três vias de transmissão: sexual, hematogênica e vertical (TV). Em áreas endêmicas, a TV tem o papel principal na cadeia de transmissão, principalmente pelo aleitamento materno. A infecção é perene e 90% dos pacientes são portadores assintomáticos, porém existe associação com uma gama de doenças inflamatórias, de dermatites inespecíficas até patologias graves e de prognóstico reservado, como a leucemia/ linfoma de células T do adulto (LTA) e a mielopatia associada ao HTLV-1, ou paraparesia espástica tropical (MAH). Até hoje não existem formas de prevenção ou tratamento eficaz para a infecção por HTLV-1/2 nem para as doenças decorrentes da mesma. Os dados sobre a infecção por HTLV-1/2 em gestantes brasileiras ainda são escassos e isolados, pois a maioria dos artigos estuda doadores de sangue, a infecção não tem notificação compulsória e o rastreio não faz parte da rotina pré-natal preconizada pelo Ministério da Saúde. O objetivo deste trabalho é reunir o conhecimento disponível sobre o tema na literatura internacional. Ainda não há estudos publicados sobre a prevalência da infecção em nosso estado, mas o projeto "Estudo da prevalência e da transmissão vertical de HIV, HTLV e Sífilis em gestantes da Região Metropolitana do Rio de Janeiro", de professores da UERJ com financiamento da FAPERJ, identificou prevalência de 0,66% em parturientes na região metropolitana do Rio de Janeiro. Com base neste resultado, foi proposta a inclusão do exame para o HTLV na rotina pré-natal do HUPE/ UERJ.O melhor conhecimento sobre a prevalência de HTLV-1/2 em gestantes nas diferentes regiões brasileiras é fundamental para orientar a elaboração de políticas educacionais, sociais e de saúde pública que possam conscientizar a população sobre a doença e reduzir as taxas de transmissão vertical.
