Premature ovarian insufficiency – aetiopathology, epidemiology, and diagnostic evaluation

Abstract: Premature ovarian insufficiency (POI) is defined as a cessation of ovarian function before the age of 40 years. It is associated with hypoestrogenism and loss of residual follicles, both of which lead to menstrual abnormalities, pregnancy failures, and decreased health-related quality of life. The prevalence of POI is estimated at 1% in the general population. Current European Society of Human Reproduction and Embryology (ESHRE) diagnostic criteria include: amenorrhoea or oligomenorrhoea for at least four mont… Show more

“…The common dominator characterizing patients presenting with ovarian insufficiency is that they typically are unable to ovulate normally since they present with reduced number of stimulable primordial follicles [2]. The wide pallet of factors affecting ovarian function, ranging from advanced maternal age, to genetic, iatrogenic, or environmental factors, may add another level of complexity toward efficient management [3,4].…”

“…However, prevalence varies depending on population characteristics with such factors as ethnicity. Indeed, the frequency is higher among Caucasian and African than Asian women [6]. Although the definition of POF is incomplete, the European Society of Human Reproduction and Embryology (ESHRE) working group recommends as diagnostic criteria [4]: Oligo/amenorrhea for at least 4 months and FSH > 25 IU/L at twice 4 months apart.…”

“…Climacteric symptoms are less common in patients with primary amenorrhea, suggesting that they are mainly due to a cessation of estrogen excretion rather than estrogen deficiency [6].…”

“…However, the etiopathogenesis of the disease in most cases remains unexplained. Factors contributing to POI include genetic abnormalities, metabolic disorders, autoimmunity, iatrogenic causes and environmental factors amongst many others [4].…”
