Objectives: Neuropathic pain (NeP) is a prevalent, disabling, multi-dimensional condition with significant morbidity; however there appears to be a variable approach in the use of outcome measures in NeP trials. A search of systematic reviews of interventional randomized controlled trials for NeP was undertaken to investigate the range and types of outcome measures employed to determine treatment effects. Methods: Keywords and MESH searches were conducted in five electronic databases from inception to 31st January 2012. Full text English language reviews based on various acute and chronic NeP conditions were included. Two independent reviewers screened papers for inclusion, extracted data, and assessed the quality of reviews. Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines were used to critically appraise the reviews.

Sin duda una de las intervenciones que más reclaman los pacientes hospitalizados es aquella que va dirigida a aliviar el dolor. El presente documento pretende dar algunas pautas propuestas por el doctor Víctor Hugo Forero en una excelente revisión que hizo acerca del tema para la Clínica Juan N. Corpas. A continuación, y con su permiso, trataremos de hacer una presentación diferente, que permita a nuestros lectores, pero en especial a nuestros estudiantes, enfrentar tal situación con más humanidad. El anhelo y la propuesta es que quienes lean este documento estén dispuestos a imponerse el lema de "Aliviar el dolor con piedad". Die diabetische Neuropathie ist eine der häufigsten und schwerwiegendsten Komplikationen des Diabetes mellitus. Sensorische Ausfälle, infizierte Fußulzera und Schäden an autonomen Nerven rauben Betroffenen oft viele Jahre Lebenserwartung. Daher ist von Anfang an Aufmerksamkeit gefragt. Neuropathische Schmerzen haben 10-20% der Diabetespatienten. Hier gilt es, den Schmerzcharakter herauszuarbeiten und geeignete Medikamente individuell zu titrieren.Der Diabetes mellitus ist eine sich weltweit rasch ausbreitende Sto wechselerkrankung, die mit vielen Komplikationen einhergehen kann. Die Prävalenz der diabetischen Polyneuropathie wird bei Diagnose der Sto wechselerkrankung mit 8% angegeben und steigt im weiteren Verlauf der Erkrankung auf bis zu 50% [1,4]. Nach neueren Untersuchungen kann bereits in prä-diabetischen Stadien der Erkrankung eine erhöhte Inzidenz neuropathischer Störungen gefunden werden [5]. Als weitere Risikofaktoren für die Entwicklung einer Polyneuropathie gelten eine viszerale Adipositas (Insulinresistenz), die Körpergröße, das Alter, der Bluthochdruck, Nikotin und Alkohol.Als häu gste Form der diabetischen Polyneuropathie ist die distale symmetrische sensomotorische Polyneuropathie sowie die autonome Neuropathie vorzu nden. Die periphere sensomotorische diabetische Polyneuropathie kann schwere trophische Veränderungen wie die Ausbildung neurotrophischer Ulzera oder einer Neuroarthropathie der unteren Extremität nach sich ziehen. Hierbei ndet man in ca. 50% eine asymptomatische periphere Polyneuropathie mit sensomotorischen Ausfällen, während die anderen Patienten sich durch Schmerzen, Parästhesien, Dysästhesien und Taubheitsgefühl zu erkennen geben. Das klinische Erscheinungsbild hängt hierbei stark vom Befall unterschiedlicher Nervenfaserpopulationen ab, wobei die Schädigung großkalibriger Nervenfasern mit einer gestörten Propriozeption und Berührungsemp ndung, der Befall kleinkalibriger Nervenfasern mit einer verminderten Schmerz-und Temperaturwahrnehmung, Parästhesien, und neuroapathischen Schmerzen einhergeht.
