Predictive Value of Silent Myocardial Ischemia for Cardiac Events in Diabetic Patients

Abstract: OBJECTIVE -Silent myocardial ischemia (SMI) in asymptomatic subjects with no history of myocardial infarction or angina is a frequent condition in diabetic patients. The aim of the study was to examine the predictive value of SMI for cardiac events in a multicenter cohort and to determine whether this value is higher in patients with a particular clinical profile. RESEARCH DESIGN AND METHODS-A total of 370 asymptomatic diabetic patients with at least two additional cardiovascular risk factors was recruited in … Show more

“…[331][332][333][334][335][336][337] In 1 multicenter study of 370 asymptomatic persons with diabetes recruited from departments of diabetology, 335 abnormality was defined as a fixed or reversible perfusion defect or a positive stress ECG. These abnormalities (compared with patients with normal study results) were associated with a 2.9-fold (1.3 to 6.4) higher risk for cardiovascular events in patients Ͼ60 years of age but not for those Ͻ60 years of age.…”

“…On the other hand, predictive algorithms created from diabetic cohorts have shown that age is a strong predictor of IHD, but little is known about the absolute risk of these events in younger people with DM. 23 Moreover, the appropriateness of existing age thresholds for the identification of people with DM who are at high risk of CVD is not known. Furthermore, in the present study, gender differences were not observed in relation to the incidence of events.…”
