Posterior Segment Complications of Endothelial Keratoplasty

Abstract: Endothelial keratoplasty is the most commonly performed type of corneal transplant in the United States, currently accounting for 60% of total grafts. The anterior segment complications are well documented; however, the posterior segment complications may not be as familiar to the anterior segment surgeon. These include endophthalmitis, cystoid macular edema (CME), choroidal detachment, graft dislocation to the posterior segment, and retinal detachment. CME is the most common postoperative retinal complication… Show more

“…The incidence of SCH related to endothelial keratoplasty has been estimated to be about 0.05% [50, 51]. Minor fluctuations in IOP observed in Descemet Stripping Automated Endothelial Keratoplasty (DSAEK) and Descemet Membrane Endothelial Keratoplasty (DMEK) have been shown to reduce the risk of SCH compared to open-sky surgical approaches [6, 52, 53]. Table 1 shows various studies of SCH in different types of keratoplasties.…”

“…Postsurgical macular edema (PSME) may complicate several intraocular procedures, notably surgery for glaucoma, corneal transplant and, most commonly, cataract extraction [1][2][3][4][5]. It has been recognized as the most common cause of visual deterioration after uncomplicated cataract surgery, with rates ranging from 0.1 to 3.8%, and it is specifically known as Irvine-Gass syndrome.…”

“…1 Endothelial transplants may lead to posterior segment complications including endophthalmitis, macular edema, retinal detachment, and suprachoroidal hemorrhage. 2 Graft detachment is one of the most common complications of this procedure. 1 However, detachment with graft dislocation into the vitreous cavity is rare, 2,3 and surgical management is required to prevent permanent vision loss because of the resulting inflammation and potential retinal detachments.…”
