Positive surgical margins after radical prostatectomy: What should we care about?

Abstract: Over a 5-year follow-up, only one-third of patients experienced BCR. It might be reasonable to postpone adjuvant radiotherapy for patients with PSM and undetectable PSA after RP. Tumor volume and PSM length were associated with BCR and should be taken into account in the postoperative treatment management.

“…PSM is a frequent situation encountered after radical prostatectomy (RP) for localised PCa with an occurrence ranging from 6 to 41% [ 9 , 26 , 27 ]. The incidence of PSM depends on various factors, including tumour biology, patient characteristics, pathological assessment method, and surgical technique [ 28 ].…”

“…Preoperative and postoperative assessment of continence and EF was done by means of self-administered questionnaires filled by the patient. In order to provide clear data regarding the effect of ePLND on continence and EF recovery and to avoid any confounding factors, the following were excluded from the analyses: neoadjuvant/adjuvant therapy, positive lymph node disease and positive surgical margins [12]. There are hints in the literature that phosphodiesterase type 5 inhibitors (PDE-5i) may adversely impact biochemical recurrence (BCR) after RP [13].…”

“…SM positivity is defined only if cancer cells are detected on the surface of the RP specimen during microscopic evaluation [8]. SM positivity occurs with a range from 6 to 41% after RP for localized PCa [9-11]. In the current study, SM positivity was seen in 33.8% of our population.…”
