Plantar sensory threshold in the ulcerative foot

Abstract: Pressure threshold measurements were made using a set of three Semmes-Weinstein monofilaments on 132 plantar u\cer sites in 72 leprosy patients and 45 plantar u\cer sites in 28 diabetic patients. The most common sites of u\ceration were the great toe and first metatarsal head. No patient could feel monofilaments smaller than 6•\0 (75 g). The next smallest filament, 5•07 (10 g), was identified as the level of protective sensation in leprosy patients who customari1y use fo otwear. Similar results were shown in a… Show more

“…These are semi-quantitative, reliable measurements, complementary to each other, with proven predictive value for the development of clinical problems, such as foot ulcers and the need for amputation (Goldberg and Lindblom, 1979;Bloom et al, 1984;Birke and Sims, 1986;Sosenko et al, 1990;Kumar et al, 1991;Veves et al, 1994;Young et al, 1994;Valk et al, 1997). Both were applied 6 times to two locations on both feet.…”

“…Nylon monofilament test is a simple test to diagnose patient's risk for the formation of foot ulcers due to peripheral sensory neuropathy [7]. The test is unusual and there is present risk of ulcers formation if the patient can not feel the touch of an monofilament when pressed on foot with only so much pressure that can bend filament [8].…”

“…Pressure sensitivity is inferred by asking individuals to close their eyes and applying nylon monofilaments calibrated to deliver predetermined forces to ascertain the minimum perceivable force applied to the skin. Within the context of hand rehabilitation, absolute sensitivity thresholds for the fingertips and sole of the foot have been reported as 0.63g and 3.5g respectively [78]. Sensitivity to light touch is also used as a screening tool for detecting neuropathic damage.…”
