Pilomatrix Carcinoma of the Scalp

Abstract: A matrical carcinoma (pilomatrix carcinoma) of the scalp is described--the first reported case in this site. This malignant hair follicle tumour must be distinguished histologically from benign pilomatricoma and proliferating trichilemmal tumour, which can have similar features.

“…In the presence of Wnt ligand, b-catenin is not phosphorylated and translocates to the nucleus and acts with the lymphocyte enhancer factor/T-cell factor family of transcription factors to activate transcription of target genes. 3,4,[6][7][8]10,11,13,16,17,21,24,25,27,29 The main differential includes proliferating pilomatrixoma, aggressive pilomatrixoma, and basal cell carcinoma with matrical differentiation (Table 2). 2,4,7,9,10 However, aneuploidy has been observed in few cases of pilomatrix carcinoma and may be of value in discriminating these neoplasms in the future after further detailed analysis.…”

“…35 Basal cell carcinoma with matrical differentiation will often display characteristic features such as peripheral palisading, retraction artifact, and a surrounding desmoplastic stroma. 9,24,29 Squamous cell carcinoma may contain cytologic features similar to the basaloid cells of pilomatrix carcinoma but lack ghost cells. 3 In addition, staining with b-catenin will reveal a cytoplasmic or membranous positivity versus both nuclear and cytoplasmic staining with pilomatrix carcinoma.…”

“…In 1993, a series of 20 patients with pilomatrix carcinoma was reported and 23 other patients in the literature were reviewed 4 . There are a further 36 cases in the world literature 5,9–40 . It has been disputed whether these 79 reported cases are all true carcinomas and some may be benign pilomatrixomas with atypical histology or behaviour 6 .…”

“…Translation of articles in Spanish, French, Italian, German, and Chinese was performed using Google translate (http://translate.google.com) or native speakers of the language. We identified 78 publications worldwide reporting a total of 135 distinct individual cases of pilomatrix carcinoma …”
