Adrenocortical activity was studied in 19 emaciated women with anorexia nervosa. Relative to body size the patients' mean cortisol production rates of 0.591 mg/kg/day and 16.4 mg m2/day were significantly elevated compared to those of 0.322 mg/kg/day and 11.4 mg/m2/day for age and sex matched normal controls (p less than 0.001 and 0.02, respectively). The 24-hr mean plasma cortisol concentration in 18 patients was 10.6 microgram/dl and was significantly higher than that of the controls (6.8 microgram/dl, p less than 0.001). The average excretion of urinary free cortisol in nine patients was 205 microgram/day, significantly greater than that of 65 microgram/day in the controls (less than 0.01). Three patients underwent overnight suppression with 1 mg of dexamethasone and had inadequate responses. These data suggest that cortisol production is excessive in emaciated patients with anorexia nervosa due to a disturbance of the hypothalamic-pituitary mechanisms regulating adrenocortical function. The excessive adrenal activity may reflect certain psychobiological disturbances as well as the effects of malnutrition.

Abstract. This paper reports on a comparative study on a synthetic depot-fi 1 2a_ tetracosactide (Cortrophine-S-Depot Organon | and an extractive ACTH-depotpreparation (Cortrophine-Z Organon| the factors studied were efficiency, dose relation, and depot effect. The parameter used was the excretion of free cortisol in the urine. Assuming a dose relation of 100 I.U. of extractive ACTH equal to 1 mg of the synthetic preparation, our results show, that the synthetic preparation evokes a stimulative effect which equals 70.2~ of that of the extractive ACTH. The difference was especially observed in the decreasing phase of the depot effect. The true dose relation calculated with regard to the parameter used, was 100 I.U. extractive ACTH to 1.42 mg of the synthetic tetracosactide. However, it has to be considered whether the difference between the two preparations regarding the effect of stimulation is in fact a question of unequal doses or a question of the stability of the depot itself. Zusammen/assung. In der vorliegenden Untersuchung werden die Wirksamkeit, die Dosisrelation und der Depoteffekt eines synthetischen fil-2%etracosactids (Cortrophine-S-Depot Organon | und eines nach herkSmmlicher Weise hergestellten Depot-ACTH-PrS~parates (Cortrophine-Z Organon | vergliehen. Als Parameter gilt die Ausseheidung des freien Cortisols im Harm Bei einer angenommenen Relation yon 100 IE Cortrophine-Z gleich 1 mg Cortrosyn-Depot zeigt dos synthetische Pr~parat bei vollwertiger adrenocorticotropcr Wirkung einen Gesamtstimulationseffekt yon 70,2% desjenigen des herkSmmlichen Priiparatcs. Dabei ist der Initialphasencffckt bei beiden Pr~paratcn gleich. In der sogcnannten Stimulationserhaltungsphase und in der Abklingphase des Depotcffektes zeigen sich Untersehiede zugunsten des herk6mmlichen Pr/iparates. Es errechnet sich eine Dosisrelation yon 100 IE Cortrophine-Z gleich 1,42 mg Cortrosyn-Depot, wobei diskutiert wird, ob es sich hier in der Tat um ein Dosisproblem oder um ein Problem der Depotstabilit~t handelt. Summary:A method is decribed for the simultaneous assay of non-conjugated androstenedione, dehydroepiandrosterone, testosterone, Cortisol, aldosterone and 18-hydroxycorticosterone in urine. The method involves solid-phase extraction, automatic high performance liquid chromatography and subsequent radioimmunological quantitation of the individual steroids. Excretion rates of these urinary free steroids were determined in normal males and females. There were no significant sex differences in excretion rates, although both urinary free testosterone and dehydroepiandrosterone were distinctly Iower in females than in males. Representative measurements of the excretion rates of patients with Cushing's disease, Addison's disease, ectopic corticotropiri syndrome and hirsutism were made. The present method has been shown to be well suited for routine purposes. Its final diagnostic significance for monitoring alterations in glucocorticoid, mineralocorticoid and androgenic activity of the adrenai cortex has yet to be explored.
