Pharmacokinetics and Pharmacodynamics of the Nitroimidazole Antimicrobials

Abstract: Metronidazole, the prototype nitroimidazole antimicrobial, was originally introduced to treat Trichomonas vaginalis, but is now used for the treatment of anaerobic and protozoal infections. The nitroimidazoles are bactericidal through toxic metabolites which cause DNA strand breakage. Resistance, both clinical and microbiological, has been described only rarely. Metronidazole given orally is absorbed almost completely, with bioavailability > 90% for tablets; absorption is unaffected by infection. Rectal and in… Show more

“…The drug regimen should be based susceptibility testing using biopsies from the patient, stool specimens, or more commonly by knowledge of the success rates of different therapies in the local area and in one's practice. Other considerations include cost and availability of drugs and should take into account the difference in pharmacokinetics and pharmacodynamics of the drugs available [25][26][27][28][29] .…”

“…Hydroxy metabolite (MTZOH) and the parent drug are biologically active, where MTZOH has 30-60% of MTZ biological activity. MTZOH has a longer half-life (12 h) and contains nearly 50% of the antitrichomonal activity of metronidazole [4,12]. With reference to the above data, we decided to examine the major metabolite of metronidazole.…”
