Rationale: The differential diagnosis of conditions manifesting as bone and joint pain is complex. Although many individuals with acute leukemia experience bone pain, lumbosacral pain as an early feature of acute lymphoblastic leukemia (ALL) is rare. Patient concerns: Here we report a case of an adult who presented with a 7-month history of persistent lumbosacral pain which had become more severe during the previous month. Diagnoses: Prior to referral, his full blood count revealed no abnormalities, and a computerized tomography scan revealed mild bone hyperplasia of his lumbar vertebrae, with disc herniations of L3–S1. His blood biochemistry and urinary test results had been normal. After referral to our clinic, tests of the morphology, immunology, cytogenetics, and molecular biology of his bone marrow led to a diagnosis of MLL-AF4 fusion positive B-cell ALL. Interventions: Prior to his referral, he had been treated with painkillers by local doctors. The painkillers initially provided pain relief, but their effect wore off over time. After diagnosis, he was started on an adult ALL chemotherapy protocol. Outcomes: His symptoms resolved within a week of starting chemotherapy. At his most recent assessment, 10 months after diagnosis, he was on maintenance chemotherapy and in remission. Lessons: This case illustrates that prolonged lumbosacral pain may be a symptom of a life-threatening condition, rather than only attributable to chronic inflammation or disk herniations. Therefore, clinicians need to pay attention to subtle differences in the clinical presentation of patients with lumbosacral pain.

Background: Acute leukemia is a malignant, rapidly progressive disease of the bone marrow and blood that most commonly occurs in children. Abnormalities of blood caused by leukemia and also accumulation of leukemic cells in different parts of the oral cavity cause a wide spectrum of oral manifestations. Objectives: The current study aimed to estimate the incidence of leukemia among children in Tehran and evaluate oral manifestations of the disease as potential diagnostic markers. Patients and Methods: In this retrospective cross-sectional study, medical records of children younger than 12 years referred to Mofid hospital, Ali-Asghar hospital and Children's medical center from 1997to 2011 were retrieved from the hospital archives and evaluated. Records of 100 patients with leukemia were randomly selected for evaluation. The obtained data were transferred to the questionnaires. Data were analyzed using SPSS version 11.5 and Chi-square test. Results: Of the 3,789 children younger than 12 years referred to the hematology department of Tehran pediatric hospitals from 1997 to 2001, 1,372 had acute leukemia, put of which 94% had acute lymphoblastic leukemia and 5% had acute myeloid leukemia. Boys aged 5 -10 years made up 69% of the acute leukemia cases. The most common oral manifestation was gingival bleeding and petechiae. The most important disease other than leukemia was anemia in the patients. Conclusions:The present study highlights the role of dentists to detect oral manifestations of leukemia and their possible role in early diagnosis of patients with the disease.

Introdução: A leucemia é uma patologia com modificações malignas nas células-tronco hematopoiéticas, podendo gerar sinais e sintomas no organismo do paciente. Objetivo: Verificar as manifestações orais de leucemia no momento do diagnóstico. Método: Foi realizada uma revisão de literatura integrativa em maio e junho de 2018, a partir das bases de dados PubMed e BIREME, por meio das palavras cadastradas no MESH e no DeCS (Descritores em Saúde), respectivamente, em inglês leukemia AND diagnosis AND oral manifestations. Foram incluídos estudos avaliando as alterações bucais em pacientes leucêmicos no momento do diagnóstico, publicados nas bases citadas, nas línguas portuguesa e inglesa, com qualquer desenho de estudo, exceto revisão de literatura narrativa e artigos publicados entre 1950 a 2018. Foram excluídos os artigos que não abordam o tema do estudo, que incluíram outros indivíduos, além de pacientes leucêmicos, com ausência de manifestações orais em pacientes leucêmicos, e artigos não encontrados em sua versão completa. Resultados: Foram encontrados 353 artigos; destes, 318 foram excluídos. Assim, restaram 35 artigos. Conclusão: Conforme descrito nos artigos encontrados, o cirurgião-dentista tem grande importância no momento do diagnóstico da leucemia, já que as primeiras manifestações dessa doença ocorrem na cavidade oral, tais como sangramento gengival, hiperplasia, inchaço gengival, ulceração oral e petéquias.
