Emerging Applications of Drug Delivery Systems in Oral Infectious Diseases Prevention and Treatment

The oral cavity is a unique complex ecosystem colonized with huge numbers of microorganism species. Oral cavities are closely associated with oral health and sequentially with systemic health. Many factors might cause the shift of composition of oral microbiota, thus leading to the dysbiosis of oral micro-environment and oral infectious diseases. Local therapies and dental hygiene procedures are the main kinds of treatment. Currently, oral drug delivery systems (DDS) have drawn great attention, and are considered as important adjuvant therapy for oral infectious diseases. DDS are devices that could transport and release the therapeutic drugs or bioactive agents to a certain site and a certain rate in vivo. They could significantly increase the therapeutic effect and reduce the side effect compared with traditional medicine. In the review, emerging recent applications of DDS in the treatment for oral infectious diseases have been summarized, including dental caries, periodontitis, peri-implantitis and oral candidiasis. Furthermore, oral stimuli-responsive DDS, also known as "smart" DDS, have been reported recently, which could react to oral environment and provide more accurate drug delivery or release. In this article, oral smart DDS have also been reviewed. The limits have been discussed, and the research potential demonstrates good prospects.Molecules 2020, 25, 516 2 of 29 problems. For example, systemic antibiotics such as tetracycline, beta-lactam antibiotics, nitroimidazoles have been used, especially in cases of periodontal diseases and peri-implantitis [12]. However, systemic antibiotics could cause problems like drug resistance, dysbacteriosis, and systemic side effects [13,14]. The antibacterial effect is also limited as very little could arrive at the oral lesion area after systemic circulation [15][16][17]. Fluoride in drinking water has been used for the prevention of dental caries but might cause excessive intake, leading to fluorosis [18]. Therefore, local drug therapy is now more considered for oral infectious diseases [19,20]. However, the conventional forms of local therapy, like drug suspension or rinse of anti-infection agents, could be easily washed off and thus could not last long in the oral cavity. Complex local lesions like deep periodontal pockets and teeth fissure are also difficult to reach. In order to improve the effect of prophylaxis and treatment, more precise targeting therapy is quite essential [21,22]. Therefore, drug delivery systems have drawn great attention in recent decades in oral infectious diseases. Drug delivery systems (DDS) are devices that can transport and release the therapeutic agents or bioactive substances to certain sites at certain rates in vivo [23,24], usually composed of the carriers and associated therapeutics [25]. Abstract. Development of nanocomposite coatings for dental implants is one of the most up-to-date and promising scientific and practical areas of focus. Numerous studies on modification problems of surfaces of implants in order to accelerate their osteointegration and achieve a high long-lasting clinical result are conducted in the world. As they remain foreign in the living body, implants should be made of special classes of materials with a high level of biocompatibility. It is these requirements that are satisfied by hydroxylapatite (HA) Ca 10 (PO 4 ) 6 (OH) 2 , the main inorganic component of the osseous tissue. HA-based coatings are effective for providing osteointegration of metal implants. But it is known that about 2 % of implants do not facilitate early osteointegration, the total level of their survival failures averages 7.7 % during five years. The key role in cases of such failures is played by diseases with a microbial genesis (known as peri-implant diseases), they eventually resulting in a loss of the implant. In view of the above facts, the urgency of development of HA coatings for dental nanocomposite implants, which are functionalized with biologically activated molecules having an antimicrobial effect, does not cause

“…Nesse ensaio, o uso adjunto do laser ao tratamento convencional não demonstrou influência positiva adicional na cicatrização periimplantar quando comparado ao grupo controle, além de ter sido constatado uma maior perda óssea marginal no grupo exposto ao laser após 06 meses do tratamento. | Aim:To evaluate and discuss the application of laser in the treatment of periimplantitis through a review of the literature. Methods: An electronic search in the PubMed database in search of publications between 2012 and 2017. Out of the total of 26 results, 17 publications were chosen which were complemented by other references relevant to the work. Results: According to the literature, Diode, Nd:YAG, Er:YAG and CO 2 lasers have been reported as a viable therapeutical method for periimplantitis because they appear to influence the surface decontamination of the implants and improve the clinical signs of inflammation. However, no controlled clinical studies have been observed that prove its long-term effectiveness, as well as the superiority of laser therapy compared to other therapeutic methods applied in the treatment of periimplantitis. Conclusions: There is still no consensus in the literature regarding the type of laser and its configurations for periimplantite treatment. It is not possible to affirm that the laser treatment is better than the conventional therapies for the periimplantite, being important to carry out more clinical studies on this Keywords: periimplantitis, laser, sistematic review. RESUMO | Objetivo: avaliar e discutir a aplicação do laser no tratamento da periimplantite através de uma revisão da literatura. Método: pesquisa eletrônica na base de dados PubMed em busca de publicações entre 2012 e 2017. De um total de 26 resultados, foram escolhidas 17 publicações as quais foram complementadas por outras referências relevantes para a realização do trabalho. Resultados: Segundo a literatura pesquisada, os lasers de Diodo, Nd:YAG, Er:YAG e o de CO 2 têm sido relatados como um método terapêutico viável para a periimplantite, pois parecem influenciar a descontaminação da superfície dos implantes e melhorar os sinais clínicos de inflamação. Porém não foram observados trabalhos clínicos controlados que provem sua efetividade a longo prazo, bem como a superioridade da laserterapia frente aos outros métodos terapêuticos aplicados no tratamento da periimplantite. Conclusão: Ainda não existe consenso na literatura a respeito do tipo de laser e suas configurações para tratamento da periimplantite. Não é possível afirmar que o tratamento a laser é melhor que as terapias convencionais para a periimplantite, sendo importante a realização de mais estudos clínicos a respeito desse assunto.Palavras-chave: periimplantitis, laser, sistematic review.
