The neurological complications of bee venom poisoning vary from optic neuritis to pontine hematoma. However, to our best knowledge, trigeminal neuropathic pain secondary to bee sting has not been reported previously in the literature. We report the case of a 52-year-old male patient with right-sided trigeminal neuropathic pain that began a month earlier, following a honeybee sting to the right forehead. The patient was successfully treated by CT-guided percutaneous trigeminal tractotomy. The present report demonstrated that a honeybee sting may result in trigeminal neuropathic pain and CT-guided percutaneous trigeminal tractotomy is effective in the treatment of such cases.

RECUERDO HISTÓRICOLa célebre frase "Hoy dueles, mañana serás solo un recuerdo…" parecía no estar relacionada con la neuralgia del trigémino (NT) hasta la aparición de nuevas técnicas invasivas.El Papiro de Ebers (1550 a. C.) describe, con gran detalle, el empleo del opio para las cefaleas del dios Ra (1), pero es el médico griego Areteo de Capadocia (50-150 a. C.) el que realiza la primera descripción inequívoca de una migraña (2,3), "dolor en el que el espasmo distorsiona el semblante...".En 1667, John Locke describe y trata una afección facial dolorosa trigeminal con ácido sulfúrico sobre la cara de la Duquesa de Northumberland.Nicolás André (1756) describe la entidad clínica como "tic doloroso" y considera el abordaje y la destrucción directa del nervio como tratamiento. No obstante es John Fothergill, el primero en describir completamente el cuadro en 1773, ante la Sociedad Médica de Londres.En 1911, Hartel y Tapias realizan la primera alcoholización del ganglio de Gasser por vía percutánea (4), y en 1931, Kirschener sugiere la destrucción del ganglio por procedimientos no quirúrgicos, usando radiofrecuencia, aunque relamente el padre de la RF Gasser fue Sweet, mejorando resultados y reduciendo complicaciones. RECUERDO ANATÓMICOEl nervio trigémino es el de mayor grosor entre los doce pares craneales, también llamado quinto par (V par). Su nombre (del latín: trigeminus, quiere decir "trillizos") deriva del hecho de poseer tres grandes ramas/nervios principales: oftálmica (V 1 ), maxilar (V 2 ) y mandibular (V 3 ).Las ramas V 1 y V 2 son sensoriales mientras que la rama mandibular tiene componente sensitivo y motor.Es un nervio mixto, pues tiene un núcleo motor y otro sensitivo, corresponde al primer arco visceral o branquial. ConexionesLa raíz sensitiva es considerablemente mayor que la motora y está formada por prolongaciones periféricas (preganglionares) y prolongaciones centrales (postganglionares) de las neuronas unipolares situadas en el ganglio trigeminal (llamado de Gasser o ganglio semilunar). A 40-year-old man developed massive subarachnoid hemorrhage following attempted radiofrequency ablation for trigeminal neuralgia. The patient had a 10-year history of trigeminal neuralgia at the ophthalmic and maxillary branches of the left trigeminal nerve. After the procedure, the patient's pain resolved completely, but one hour later sudden onset of severe headache in the frontal region was appeared. Computed tomography (CT) scan of the cranium showed massive subarachnoid hemorrhage, and an air bubble was observed in the left cavernous sinus. Herein, we present the CT feature of this rare complication. Keywords: Radiofrequency ablation; subarachnoid hemorrhage; traumatic; trigeminal neuralgia Özet Kırk yaşındaki erkek hastada trigeminal radyofrekans rizotomi sonrası yaygın subaraknoid kanama gelişti. Hastada yaklaşık 10 yıldır süren, trigeminal sinir oftalmik ve maksiller dallarına uyar traselerde ağrı şikayeti vardı. İşlem sonrası hastanın ağrısı tamamen geçti ancak 1 saat sonra frontal bölgede şiddetli baş ağrısı oldu. Kraniyal bilgisayarlı tomografisinde yaygın subaraknoid kanama ve sol kavernöz sinüste hava dansitesi saptandı. Burada bu nadir komplikasyonun bilgisayarlı tomografi bulgularını sunmayı amaçladık.
