Pemphigoid gestationis successfully treated with intravenous immunoglobulin

Abstract: Pemphigoid gestationis (PG), also known as , is a rare autoimmune blistering disease specific to pregnancy, which usually presents in the second or third trimesters and, in 15%-25% of cases, during the immediate postpartum period.Although the ethiopathogeny of PG is not fully clarified, most patients develop antibodies against a 180 kDa transmembrane hemidesmosomal protein (BP180; BPAG2; collagen XVII). PG has a strong association with human leucocyte antigens DR3 and DR4.We report a case of a 29-year-old fema… Show more

“…If PG remains refractory to treatment, or prolonged courses of steroids are required, other agents such as intravenous immunoglobulins, dapsone, and azathioprine < 2 mg/kg/day should be considered as steroid-sparing agents [65]. Intravenous immunoglobulins (FDA pregnancy category C) have a good safety profile for the patient and fetus and should be used either in monotherapy or as adjuvant treatment [66][67][68][69][70][71][72][73][74]. Dapsone (FDA pregnancy category C) is also considered safe during gestation.…”

“…Other treatments include antihistamines, immunosuppressive drugs, plasmapheresis and intravenous immunoglobulins. [2][3][4][5] Dupilumab is a monoclonal antibody targeting the alpha subunit of the interleukin-4 (IL-4) receptor, inhibiting the IL-4 pathway of IL-13. This biotherapy has few side-effects, and its usefulness is increasingly being documented in refractory and corticosteroid-dependent bullous pemphigoid, in order to avoid the use of more potent immunosuppressant.…”
