Pediatric Segmental Eccrine Spiradenomas: A Case Report and Review of the Literature

Abstract: Eccrine spiradenoma is a rare benign adnexal tumor that usually occurs as a solitary nodule, although there are reports of multiple eccrine spiradenomas occurring in a segmental, linear, blaschkoid, or zosteriform pattern. Segmental eccrine spiradenomas have rarely been reported with onset occurring in childhood or adolescence. We describe a case of segmental eccrine spiradenomas occurring on the neck and mandible of a child.

“…4 Several investigators had studied other less invasive therapies for spiradenoma. In 2013, Gordon et al 5 in the United States reported a case of spiradenoma treated using intralesional 10 mg/mL TA injection, but there was minimal improvement. Gottschalk et al 6 treated a skin adnexal tumor with intralesional 40 mg/mL TA injection.…”

“… 2 , 4 However, the efficacy of some of these modalities has not been determined. 4 In 2013, Gordon et al 5 in the United States reported a case of spiradenoma treated using injection of triamcinolone acetonide (TA) 10 mg/mL with minimal improvement. Gottschalk et al 6 administered TA 40 mg/mL in one case of adnexal tumor with a 75% reduction in lesion size.…”

“…Radiotherapy or CO 2 laser are alternative choices which have shown satisfactory results. Intralesional infusion of botulinum toxin A and triamcinolone has been tested providing minimal improvement and its long term efficacy remains questionable [11,12]. It is widely accepted that future studies are needed and until then surgical excision remains the gold standard therapy.…”

“…Ter Poorten et al 16 treated cases with small discrete lesions by surgical resection combined with CO 2 laser ablation; the treatment was effective with symptomatic relief. Trials using intralesional botulinum toxin A and intralesional triamcinolone have been used to treat multiple ES 17 . Nevertheless, additional long-term follow-up is required to determine the optimal management strategy.…”
