BACKGROUND: Psychological factors influence the development and persistence of chronic low back pain (CLBP) and may impair the psychosocial rehabilitation success. OBJECTIVE: To examine the effects of a combined pain competence and depression prevention training compared to the pain competence training alone and as well as the patients’ stages of pain on the long-term psychosocial rehabilitation success. METHODS: In this controlled multicentre study with cluster-block randomization, patients with CLBP in different stages of pain (I–III) received either pain competence training (control group, CG; n= 255) or combined pain competence and depression prevention training (intervention group, IG; n= 271; per protocol). Depressive symptoms (primary outcome), anxiety, somatization, health status, and average pain intensity (secondary outcomes) were assessed up to 12 months of follow-up. Standardised questionnaires were used to record the outcomes, which were filled out by the patients themselves. Analyses after multiple imputation (N= 1225) were conducted to validate multi- and univariate analyses of variance. RESULTS: Patients in stage of pain I and II showed significant improvements in depressive symptoms, anxiety, mental health, and average pain intensity at the 12-month follow-up, irrespective from treatment condition. CONCLUSIONS: Multidisciplinary rehabilitation seems to be appropriate for patients with CLBP in stage of pain I and II. However, patients in stage of pain III need more psychological treatments to manage their mental comorbidities.

Psychosocial comorbidity in somatic disorders is considered an important problem from the epidemiological point of view as well as from the perspective of care. Therefore, the different origins or development are etiologically discussed. In this article, an overview of the prevalence of psychiatric comorbidity of musculoskeletal disorders, using chronic back pain in somatic medical rehabilitation as an example, is given. According to the statistics of the German pension insurance fund, musculoskeletal diseases are the most common indication. In this article, the opportunities and the challenges for medical rehabilitation in managing psychosocial comorbidity are discussed. Starting with the description of medical rehabilitation in Germany, the significance of psychosocial comorbidity in medical rehabilitation and the resulting opportunities arising from this special setting to screen for psychological comorbidity are examined. These aspects are discussed based on articles from 2000-2010 in the field of orthopedic inpatient medical rehabilitation. A total of ten studies, focusing on the medical rehabilitation of chronic back pain, were identified. It was found that three papers reported data on the prevalence of psychological comorbidity and addressed diagnostic screening. This is discussed from a perspective that addresses potential barriers on the part of therapists as well as the patients. More attention should be directed to implementing the appropriate screening and follow-up care.

Immediately after therapy, a general improvement was found regarding depressiveness and pain (strong effect size) as well as regarding pain-related anxiety (medium effect sizes). Only "countertraded activity" of coping with pain did not show any improvement. The "rehabilitation status" improved significantly whereas, in contrast, the "physical health" aspect did not improve. A significant increase was found on the "pain" scale. Additionally, depressive symptoms had a strong impact on the aspects "physical health", "subjective health" and on "rehabilitation status". In particular, patients with fibromyalgia and depressiveness showed poorer outcomes regarding these aspects than patients with fibromyalgia without depressiveness. A multimodal therapy with patient education can have a positive impact on the outcome of rehabilitation.
