Comparative Studies on Patient Safety Culture to Strengthen Health Systems Among Southeast Asian Countries

Patient safety is an important issue in health systems worldwide. A systematic review of previous studies on patient safety culture in Southeast Asian countries is necessary for South Korea's partnership with these countries, especially given South Korea's assistance in strengthening the health systems of these developing countries. Studies on patient safety culture in Southeast Asian countries, published in English and Thai languages, were retrieved from computerized databases using keywords through a manual search. Data extraction, quality assessment, and analyses were performed using several tools. The review included 21 studies conducted in Indonesia (n = 8), Thailand (n = 5), Malaysia (n = 3), Vietnam (n = 2), Singapore (n = 1), and the Philippines (n = 1). They were analyzed and categorized into 12 dimensions of safety culture, and differences in response rate or scores were identified compared to the mean of the dimensions. The heterogeneous of safety culture's situation among Southeast Asian countries, both in practice and in research, can be explained since patient safety policy and its application are not prioritized as much as they are in developed countries in the priority compared to the developed countries. However, Vietnam, Cambodia, Myanmar, and Laos are the priority countries for South Korea's official healthcare development assistance in the Southeast Asia region. Vietnam, for instance, is an economically transitioning country; therefore, consolidated patient safety improvement by inducing patient safety culture in the provincial and central health system as well as strengthening project formulation to contribute to health policy formation are needed for sustainable development of the partner countries' health systems. It is recommended that more evidence-based proactive project planning and implementation be conducted to integrate patient safety culture into the health systems of developing countries, toward health policy on patient safety and quality service for the attainment of sustainable development goals in South Korea's development cooperation.

Reports of patient safety incidence at health service provider have yet been optimized. Report rates are still low and health service providers were facing obstacles in reporting incidents. Therefore, the purpose of this study was to identify obstacles in reporting patient safety incidents. A literature review was the method of choice in this study. Literature sources were obtained from the Pubmed and Ebsco Medline databases based on inclusion criteria. Based on the literature search results that have been done, we get as many as six (n = 6) articles. The obstacles that were found in reporting incidents are the negatif impact felt by the reporter, the lack of time in reporting incidents, lack of feedback, certain types of incidents reported, lack of knowledge, incidence reports were not considered as obligation, lack of clarity on who should report, lack of anonymity, and reporting system that has yet been optimized. Meanwhile, the ways to overcome these obstacles are improving and increasing report rates, giving feedback, increasing anonymity and secrecy, as well as giving the reward, education, and training for incident reports. As conclusion, obstacles in reporting incidence surely can hinder patient safety and therefore need to be resolved. Commitment from policy maker were necessary in improving patient’s safety incident reporting system. Keywords: obstacles; incidence report; patient safety ABSTRAK Pelaporan insiden keselamatan pasien di pelayanan kesehatan saat ini belum optimal. Tingkat pelaporan masih rendah, petugas kesehatan masih merasakan kendala dalam melaporkan kejadian. Oleh karena itu, tujuan dari penelitian ini adalah untuk mengidentifikasi hambatan dalam pelaporan insiden keselamatan pasien. Metode yang digunakan dalam studi ini adalah literature review. Sumber literatur didapatkan dari basis data Pubmed dan Ebsco Medline berdasarkan kriteria inklusi. Berdasarkan hasil pencarian literatur yang telah dilakukan, kami mendapatkan sebanyak enam (n=6) artikel. Hambatan pelaporan insiden yang ditemukan dalam penelitian ini adalah adanya dampak negatif yang dirasakan oleh pelapor, kurangnya waktu melaporkan insiden, kurangnya umpan balik, jenis insiden tertentu yang dilaporkan, kurangnya pengetahuan, pelaporan tidak dianggap sebagai kewajiban, kurangnya kejelasan tentang siapa yang harus melaporkan, kurangnya anonimitas, dan sistem pelaporan yang belum optimal. Sedangkan cara mengatasi hambatan atau fasilitator pelaporan insiden adalah mengembangkan dan meningkatkan sistem pelaporan, memberikan umpan balik, meningkatkan anonimitas dan kerahasiaan, serta memberikan penghargaan, pendidikan dan pelatihan tentang sistem pelaporan insiden. Sebagai kesimpulan, hambatan dalam melaporkan insiden tentunya menghambat peningkatan keselamatan pasien sehingga diperlukan upaya untuk mengatasinya. Senior nurses’ perceptions of junior nurses’ incident reporting: A qualitative study

Aim To develop an understanding of senior nurses’ ranking and perceptions of incident reporting by junior nurses. Background Nurses must be encouraged to report incidents to nursing management. It is important to ascertain how senior nurses perceive their concerns, as it is crucial to ensuring that patient safety is managed. Method Qualitative study. Four focus groups explored senior nurses’ perceptions of risks identified by nurses from a live incident reporting database. Data were analysed using framework analysis. Results Five themes emerged demonstrating the differences in opinions in relation to the classification of events by senior and non‐senior nurses. Senior nurses held the view that some junior nurses use incident reporting to ‘vent frustration.’ Conclusion There is a mismatch between senior nurses’ and junior nurses’ perceptions of safety incidents. Nurses need to develop the writing style and use language that red flags incidents when reporting incidents. Senior nurses need to create a positive culture where risk from incident reporting is used to improve patient safety and subsequently a positive work environment. Implications for Nursing Management Our research identified the need for joint training to promote a shared understanding among nurses as to how incident report should be completed to promote patient safety.
