В статье проведена оценка эффективности монотерапии и комбинированной терапии симптоматической офтальмогипертензии у пациентов с эндокринной офтальмопатией (ЭОП). Изучены показатели внутриглазного давления (ВГД) и активность течения ЭОП у 145 пациентов (290 глаз и орбит) с различными формами ЭОП. Из общего числа обследованных пациентов выделена подгруппа пациентов с впервые выявленной симптоматической офтальмогипертензией - 28 пациентов (56 глаз и орбит), 19% случаев. Учитывая активное течение ЭОП, пациентам с симптоматической офтальмогипертензией назначена противоотечная, противовоспалительная (пульс-терапия метилпреднизолоном) и местная гипотензивная терапия. На старте всем пациентам с симптоматической офтальмогипертензией назначена монотерапия селективными бета-адреноблокаторами (ББ) или аналогами простагландинов (АПГ). При отсутствии компенсации уровня ВГД пациенты переведены на фиксированные комбинации бета-адреноблокатор и ингибитор карбоангидразы (ББ/ИКА) или бета-адреноблокатор и аналог простагландинов (ББ/АПГ). Вподгруппе пациентов с симптоматической офтальмогипертензией монотерапия эффективна при повышении уровня ВГД в пределах Ме 25,5 [25; 26] мм рт. ст. (критерий Уилкоксона, р=0,0004) и среднеактивном течении процесса по шкале CAS Me 5 [4; 5] баллов. В качестве монотерапии наиболее эффективно снижают уровень ВГД селективные ББ (на 20% от исходного). При активном течении процесса (Me 6 [6; 7] баллов) и уровне офтальмотонуса свыше Ме 29 [27,5; 30,5] мм рт. ст. наиболее выраженным гипотензивным эффектом обладают фиксированные комбинации ББ/ИКА (критерий Манна - Уитни, р=0,005), они позволяют снизить уровень ВГД в комплексной терапии на 31,1% от исходного. При выборе стартовой гипотензивной терапии у пациентов с симптоматической офтальмогипертензией на фоне ЭОП необходимо учитывать не только уровень ВГД, но и активность течения процесса. The purpose of this study to compare effect of monotherapy and fixed combination therapy of symptomatic hypertension in patients with Graves orbitopathy (GO). The study included 145 patients (290 eyes and orbits) with different forms and activity of GO. In group with symptomatic ophthalmohypertension included 28 patients (56 eyes and orbits), 19% of cases. In monotherapy patients received selective beta-blocker (BB) or prostaglandin analogue (PGA). In fixed combination ophthalmic solution patients received beta-blocker/carbonic anhydrase inhibitor (BB/CAI) or beta-blocker/prostaglandin analogue (BB/PGA). In group with symptomatic ophthalmohypertension, monotherapy is effective in baseline IOP level Me 25.5 [25; 26] mm Hg (Wilcoxon test, p=0.0004) in mild activity of the process according to the CAS scale Me 5 [4; 5]. Selective BB can effective reduced IOP by 20% from baseline. In cases of symptomatic ophthalmohypertension with IOP level Ме 29 [27.5; 30.5] mm Hg and the active course of the process in CAS scale Me 6 [6; 7] fixed combinations BB/CAI (Mann - Whitney test, p=0.005) can reduce the IOP level in complex therapy by 31.1%.

Background Graves’ ophthalmopathy (GO) is one of the most common autoimmune inflammatory disorders affecting the orbit that characterized by swelling of extra ocular muscles (EOMs) and expansion of the orbital fat. Diffusion tensor imaging (DTI) could assess the microstructural integrity of tissue. We aimed at this study to assess the role of DTI in the evaluation of EOMs and orbital fat in GO and identify the relationship with disease activity. Results Case–control study included 40 patients diagnosed as Graves’ disease (20 active and 20 inactive) and 10 health control subjects underwent DTI. Low fraction anisotropy (FA) and high mean diffusivity (MD) of inferior rectus (IR), medial rectus (MR) and orbital fat in GO versus healthy control (HC), while high FA and high MD in active group versus inactive group. In order to differentiate between GO and HC; FA cutoff point of IR, MR& orbital fat were 0.46, 0.45 and 0.26 with sensitivity 98.8%,98.8% and 93.8% and specificity 95.0%, 95.0% and 85%, respectively. MD cutoff point for IR, MR and orbital fat 1.24, 1.27 and 1.275 with sensitivity 97.5%, 98.8% and 98.8% and specificity 95.0%, 95% and 95%, respectively. To differentiate between active and inactive GO; FA cutoff point of IR, MR and orbital fat were 0.35, 0.36 and 0.22 respectively with sensitivity 80.0%, 82.5% and 72.5% and specificity 95.0%, 85.0% and 65.0%, respectively. MD cutoff point for IR, MR and orbital fat were 1.58, 1.63 and 1.54 respectively with sensitivity 90.0%, 97.5% and 85.0%, and specificity 90.0%, 80.0% and 62.5%, respectively. Conclusions DTI parameters (FA and MD) of EOMs and orbital fat are considered as crucial radiological biomarkers for diagnosis of GO and could quantitatively differentiate active form inactive disease.

Endocrine ophthalmopathy (EOP) is an autoimmune progressing pathology, which in most cases is associated with Graves’ disease (GD), but also occurs in patients with autoimmune thyroiditis and euthyroidism without symptoms of thyroid disease. Despite the existence of the proven interconnection between thyroid pathology and EOP, the mechanisms of its pathogenesis have not been studied completely. The article covers the modern theories of etiology and pathogenesis, genetic predisposition to the development of EOP, the risk factors, features of the clinical course and major complications of this pathology. EOP may cause significant loss of visual acuity and lead to disability, so only the multidisciplinary approach, timely diagnosis and treatment may prevent the complications and preserve the quality of life of patients.
