Paraganglioma of the urinary bladder: A clinicopathologic spectrum of a series of 14 cases emphasizing diagnostic dilemmas

Abstract: Paraganglioma of the urinary bladder is a rare tumor and may be misdiagnosed as urothelial cancer especially on TURBT, but a high index of suspicion, careful search for the characteristic histological features and supportive immunohistochemical studies should lead to a correct diagnosis.

“…The bladder sympathetic paraganglioma, amounting to 6% of all extra-adrenal pheochromocytomas, accounts for 0.06-0.1% of all bladder tumors (2,6). Given that it arises from sympathetic nervous system-derived chromaffin cells within the bladder wall layers, its reliable idenfication may pointed out on the basis of typical histological features and, particularly, by resorting to suitable immunohistochemical assessment in addition to specific blood plasma/urine labor tests and imaging approaches (Table 1) (2,5,7,10,13,16).…”

“…13,22 In one particular study, a single case of distant metastasis developed 10 years after diagnosis. 17 Therefore, longer follow-up periods are recommended to determine the exact frequency of malignant PBPGs. Third, mutational analysis was not available for 1 SDHB-intact tumor that developed metastasis.…”
