Papain-Induced Asthma: A Man With Dyspnea From Dawn Till Dust

Abstract: A 52-year-old man, a current smoker (40 pack years) with unremarkable medical history, was referred to the outpatient pneumology clinic because of recent complaints of shortness of breath and wheezing, which were relieved by inhaled bronchodilators. Serial peak expiratory flow (PEF) measurements showed a clear rise in PEF during the weekend and a fall on the evening after the first day of the week. It also showed that evening values were always lower than morning values. During a holiday, a slow but persistent… Show more

“…Asthma could be induced by inhalation of various allergens (such as mold spores, pollen, mold, and dust mites) [1], air pollutants (such as smoke) [4], and protease allergen such as papain. The latter has been reported to cause occupational asthma in humans [5,6] and asthmalike symptoms in mice [7,8]. Asthma is one of the complex multifactorial diseases, thus several animal models were used to study their progression and pathogenesis following Int.…”

“… 3 It has well documented that papain can induce reactions through different routes: respiratory, 2 4 5 skin/conjunctival, 6 7 8 or oral. 9 Papain allergy can occur in occupational and non-occupational settings, and most of the affected subjects work in a factory, 4 5 a biochemical laboratory, 2 or a beauty salon. 6 8 Food processors may affect IgE-mediated allergic occupational diseases through halation of airborne particles, skin contact, or ingestion.…”

“…Cellular analysis of the patient's sputum, nasal lavage and tear fluid before and after the specific inhalation and nasal challenge test with papain Tabela 1. Ocena cytologiczna plwociny, popłuczyn nosowych i łez pobranych od pacjenta przed swoistą prowokacją wziewną i donosową z papainą oraz po prowokacji Papain-induced rhinoconjunctivitis and asthma Nr 1 111 to other reports [1][2][3]5], the immunoglobulin E-mediated (IgE) sensitization to papain was confirmed with a positive SPT result with a papain solution. In our case, the SICT and SNCT with papain induced allergic responses from the respiratory system and conjunctiva.…”
