Prevalence and Predictors of Poor Sleep Quality in Patients on Maintenance Hemodialysis

Purpose Majority of patients on maintenance hemodialysis complain of poor quality of sleep. Earlier studies show controversial findings with regard to the prevalence and predictors of poor quality of sleep in this group. Hence, the present study was planned. Methods Adult subjects undergoing maintenance hemodialysis were included in this study. Their demographic data, history regarding chronic kidney disease, comorbidities, and details related to hemodialysis were recorded. Sleep quality was assessed using Pittsburg Sleep Quality Index. A total of 201 patients were included in this study. Results Average age of the subjects was 54.7 ± 15.1 years and duration of kidney disease was 37.1 ± 19.8 years. 40.3% subjects were women and 50.7% of sample complained of poor quality sleep. Poor sleepers had less than 3/week dialysis (68.6% among poor sleepers vs. 21.2% among good sleepers; P \ 0.001), nighttime dialysis shift (52.9 vs. 20.2% among good sleepers; P \ 0.001), diabetes mellitus (56.9 vs. 39.4% among good sleepers; P = 0.01), and hypnotic use (P \ 0.001). Similarly, subjects with poor sleep quality were older (57.3 ± 14.3 vs. 52.1 ± 15.4 years among good sleepers; P = 0.01). Binomial logistic regression showed that less than thrice a week dialysis and schedule of maintenance hemodialysis predicted the poor sleep quality. Less than 3/week dialysis increased the odds for poor sleep by 6.6 (P \ 0.001) and afternoon and nighttime maintenance hemodialysis shift by 3.2 (P \ 0.001) and 4.6 (P \ 0.001), respectively. Conclusion Poor sleep is a common complaint in patients on hemodialysis. We found that the evening or night shift of hemodialysis and twice weekly hemodialysis worsened the quality of sleep.

OBJECTIVE:We aimed to determine the type, frequency, location, intensity and the characteristics of the pain in hemodialysis patients. MATERIAL and METHODS:Ninety-fi ve hemodialysis patients were investigated using McGillMelzack pain questionnaire. Location, nature, frequency and intensity of the pain were recorded according to this form. In addition, pain was classifi ed with regard to duration as instant, acute and chronic. RESULTS:Sixty patients (63.1%) reported a problem with pain, the others (35 patients -36.8%) reported not to have pain. When considering the relationship of pain with time; 24.1% of the patients had continuous, steady, fi xed pain and 58.6% of the patients had rhythmic, periodic, intermittent pain while 17.2% had generalized, momentary, temporary pain. When the pain was evaluated in terms of severity, it was found that 31 patients (52%) had mild and disturbing pain, while 29 patients (48%) had severe or very severe (intolerable) pain. It was also detected that 60 patients had instant pain, 25 had acute pain and 31 had chronic pain. CONCLUSION:Pain is a common and chronic problem in patients undergoing HD and it is seriously necessary to address treating the pain along with other systemic diseases in order to increase the quality of life of individuals. KEY WORDS:Hemodialysis, Pain, Quality of life ÖZ AMAÇ: Bu çalışmada; diyalize giren son dönem kronik böbrek yetmezlikli hastalarda görülen ağrının tipi, sıklığı, lokalizasyonu, şiddeti ve karakterlerinin belirlenmesi amaçlanmıştır. GEREÇ ve YÖNTEMLER:Hemodiyaliz tedavisi gören 95 hastanın ağrı şikâyetleri McGill-Melzack ağrı soru formu kullanılarak birebir görüşme yöntemi ile araştırıldı. Bu formla ağrının yeri, özelliği, zamanla ilişkisi (1. devamlı, kararlı, sabit, 2. ritmik, periyodik, aralıklı, 3. genel, anlık, geçici) ve şiddeti (1. hafi f, 2. rahatsız edici, 3. şiddetli, 4. çok şiddetli, 5. dayanılmaz) değerlendirildi. Ayrıca ağrı tipleri anlık (son 24 saate), ciddi (3 günden fazla süren işten ve uykudan alıkoyan) ve kronik (3 aydan uzun süren) ağrı olarak sorgulandı. BULGULAR:Hastaların 60'ı (%63,1) ağrıdan şikayet ederken, 35'inde (%36,8) ağrı şikayeti yoktu. Hastaların % 24,1'inde devamlı, kararlı, sabit ağrı, % 58,6'sında ritmik, periyodik, aralıklı, % 17,2'sinde genel, anlık, geçici ağrı tipine rastlanmıştır. Şiddeti açısından değerlendirildiğinde ise 31 hasta (%52) hafi f ve rahatsız edici, 29 hasta (%48) şiddetli ve çok şiddetli ağrı tespit edildi. 60 hastada anlık ağrı, 25 hastada ciddi ağrı ve 31 hastada kronik ağrı olduğu saptandı. Ağrı tiplerinin yaş, beden kitle endeksi ve diyaliz süreleriyle ilişkisi saptanmadı. SONUÇ: Diyaliz hastalarında ağrı, yaygın ve kronik bir sorun olarak karşımıza çıkmaktadır. Bu hastaların yaşam kalitelerini arttırmak için ağrı da ciddi bir şekilde ele alınmalı ve tedavi edilmelidir.

ÖzAmAÇ: Hemodiyaliz (HD) tedavisi gören hastalarda ağrının sıklığını, nedenlerini ve şiddetini incelemek amacıyla yapılmış tanımlayıcı ve kesitsel bir çalışmadır. GErEÇ ve yÖntEmlEr:Örneklemi; Uşak'ta iki özel HD merkezinde tedavi gören, araştırma sınırlılıklarına uyan ve çalışmaya katılmayı kabul eden 150 HD hastası oluşturdu. Veriler, Hasta Tanıtım Formu ve McGill-Melzack Ağrı Soru Formu kullanılarak toplandı ve ortalama, t testi, Mann-Whitney U testi ve Kruskal Wallis H testleri ile değerlendirildi.BUlGUlAr: HD hastalarının neredeyse hepsinin (%96) ağrı hissettiği, büyük bir çoğunluğunun hafif ve rahatsız edici düzeyde, geriye kalanların ise şiddetli, çok şiddetli ya da dayanılmaz düzeyde ağrı yaşadığı saptandı. Ağrının baş, abdomen, alt ve üst ekstremiteler gibi vücudun farklı alanlarına yayıldığı; hastaların ağrı ile başetmede ise, farmakolojik ve non-farmakolojik tedavileri tercih ettikleri görüldü. Data collection was achieved using a Patient Information Form and the McGill-Melzack Pain Questionnaire, and evaluation was made using means, the t test, and the Mann-Whitney U and Kruskal-Wallis H tests. rESUltS:It was found that nearly all HD patients (96%) experienced pain that was light or at the level of discomfort for most but at a severe, very severe or unbearable level for the rest. The pain spread from various regions of the body such as the head, abdomen, and upper and lower extremities. Patients chose both pharmacological and non-pharmacological treatments to cope with the pain. We found that gender, a weekly frequency of HD, and living alone were factors that influenced the severity of pain.cOnclUSIOn: Almost all patients undergoing HD experience pain and holistic care is important to relieve this pain. Effective holistic treatment necessitates a specific pain diagnosis, planned treatment and care, localizing the pain and determination of the factors that affect it.
