ÖzAmAÇ: Hemodiyaliz (HD) tedavisi gören hastalarda ağrının sıklığını, nedenlerini ve şiddetini incelemek amacıyla yapılmış tanımlayıcı ve kesitsel bir çalışmadır. GErEÇ ve yÖntEmlEr:Örneklemi; Uşak'ta iki özel HD merkezinde tedavi gören, araştırma sınırlılıklarına uyan ve çalışmaya katılmayı kabul eden 150 HD hastası oluşturdu. Veriler, Hasta Tanıtım Formu ve McGill-Melzack Ağrı Soru Formu kullanılarak toplandı ve ortalama, t testi, Mann-Whitney U testi ve Kruskal Wallis H testleri ile değerlendirildi.BUlGUlAr: HD hastalarının neredeyse hepsinin (%96) ağrı hissettiği, büyük bir çoğunluğunun hafif ve rahatsız edici düzeyde, geriye kalanların ise şiddetli, çok şiddetli ya da dayanılmaz düzeyde ağrı yaşadığı saptandı. Ağrının baş, abdomen, alt ve üst ekstremiteler gibi vücudun farklı alanlarına yayıldığı; hastaların ağrı ile başetmede ise, farmakolojik ve non-farmakolojik tedavileri tercih ettikleri görüldü. Cinsiyetin, haftalık HD sıklığının ve yalnız yaşamanın ağrının şiddetini arttıran faktörler olduğu bulundu.SOnUÇ: HD tedavisi alan hastaların neredeyse hepsi ağrı hissetmekte ve ağrının dindirilmesi için, bütüncül bakımın sağlanması önem arz etmektedir. Etkin, bütüncül bir bakım ise, ayrıntılı ağrı tanılamasını, tedavi ve bakımının planlanmasını, ağrı lokalizasyonlarını ve etkileyen etmenleri belirlemeyi gerektirmektedir.AnAhtAr SÖzcüKlEr: Hemodiyaliz, Ağrı, Son dönem böbrek yetersizliği ABStrAct OBJEctIVE: We evaluated the frequency, causes and severity of pain in patients undergoing Hemodialysis (HD) treatment in this descriptive cross-sectional study. mAtErIAl and mEthOdS:The study sample consisted of 150 HD patients undergoing treatment at two HD centers in Uşak, Turkey. Data collection was achieved using a Patient Information Form and the McGill-Melzack Pain Questionnaire, and evaluation was made using means, the t test, and the Mann-Whitney U and Kruskal-Wallis H tests. rESUltS:It was found that nearly all HD patients (96%) experienced pain that was light or at the level of discomfort for most but at a severe, very severe or unbearable level for the rest. The pain spread from various regions of the body such as the head, abdomen, and upper and lower extremities. Patients chose both pharmacological and non-pharmacological treatments to cope with the pain. We found that gender, a weekly frequency of HD, and living alone were factors that influenced the severity of pain.cOnclUSIOn: Almost all patients undergoing HD experience pain and holistic care is important to relieve this pain. Effective holistic treatment necessitates a specific pain diagnosis, planned treatment and care, localizing the pain and determination of the factors that affect it.
The purpose of the present study was to examine the knowledge and attitudes of Turkish nursing students about pain management. Methods: The participants in this cross-sectional study were 190 nursing students who were studying at a nursing faculty in western Turkey. The research data were collected between March and June of the academic year 2015-2016, using a Student Description Form and the Nurses' Knowledge and Attitudes Survey Regarding Pain (NKASRP). Results: The mean age of the students in the study sample was 23.06±1.44 years. In all, 83.7% were female, 96.8% were unmarried, 94.7% were working as a nurse, and all had social security. The mean score of the NKASRP was 15.85±3.25%. A comparison of the knowledge scores with student characteristics revealed that only the status of current employment as a nurse demonstrated a significant difference. No significant difference was detected between the NKASRP score and variables such as age and the score of the worst pain ever experienced (p>0.05). Conclusion: It was determined that the students had a generally poor level of pain knowledge. It will improve the quality of life of patients if pain management is given a greater place in undergraduate education.

ÖZETNiemann Pick Sendromu (NPS), sfi ngomiyelin metabolizma bozukluğuna bağlı olarak lipozomlarda sfi ngomiyelin ve kolesterol depolanmasına bağlı genetik (otozomal resesif) geçişli bir hastalıktır. Klinik belirti ve bulgular arasında disfaji, motor fonksiyon kaybı, hepatosplenomegali, tekrarlayan solunum yolu enfeksiyonları, nöbet, mental retardasyon, spastisite, miyoklonik nöbetler ve ataksi bulunmaktadır fakat hastalığın tipine göre de değişiklik göstermektedir. Gözlenen patolojiye göre özel tedavi gerekmektedir. Bu grup hastada patolojinin yüksek prevalansından dolayı, çoğunlukla genel anestezi gerektiren cerrahi girişimlere ihtiyaç duyulmaktadır. NPS'lu hastalarda anestezistler bazı sorunlar yaşamaktadırlar. Bunlardan biri hepatosplenomegaliye bağlı ventilasyon, diğeri ise entübasyon zorluğudur. Ayrıca kronik antikonvülzan ilaç kullanımı bazı anestezik ilaç metabolizmalarını etkileyebilir. Karaciğer fonksiyon testleri yüksek, trombosit sayısı düşüktür. Burada nadir görülen bir hastalık olan Niemann Pick Sendrom'lu bir kız çocukta acil olarak uygulanan anestezi yaklaşımı, zorlukları ve post-op bakımı sunulmuştur. NPS'li hastalarda anestezistler ventilasyon ve entübasyon zorluğu yaşamaktadırlar. Bu durumda düşük tidal volüm ve yüksek frekans ile ventilasyon yapılması gerekir. Karaciğer hasarı yapabileceğinden anestezik ajanlar dikkatli kullanılmalıdır. Sonuç olarak, tüm bu patolojiler akılda tutularak bu hastalara anestezi yaklaşımda dikkatli olunmalıdır. The clinical symptoms and signs include dysphagia, loss of motor function, hepatosplenomegaly, recurred respiratory infections, seizure, mental retardation, spasticity, myoclonic jerks and ataxia, but vary depending on the type of this disease. According to the observed pathology, patients require specialized therapy. Due to the high prevalence of the pathology in this group of patients, surgical interventions requiring general anaesthesia are common. Anesthetists have some diffi culties with this group of patients. One of them is diffi cult ventilation because of hepatosplenomegaly and the other is diffi cult intubation. The metabolism of some of the anesthetic agents may be affected due to chronic use of anticonvulsant agents. Liver enzymes are elevated and platelet counts are reduced. Here we report an anesthesia management, diffi culties and post-op follow up in a female child having NPD. Anesthetists have some diffi culties in ventilation and intubation with NPD patients. MSTN genotyping revealed that only 3.9% (6/152) of participants had the MSTN K153R heterozygous mutation. Despite the detection of this mutation in the study group, no relationship was found between this mutation and sarcopenia.
