Ovine panel reactive antibody assay of HLA responsivity to allograft bioengineered vascular scaffolds

Abstract: These studies with an ovine modified panel reactive antibody assay confirmed minimal immune allosensitization to transplanted decellularized tissue patches. Qualifying criteria for putative tissue-engineered scaffolds should include minimal recipient panel reactive antibody response.

“…One of the major advantages of decellularized matrices is their reduced immunogenicity, which has been reported in heart valve, 10 blood vessel, 26,27 cardiac defect, 28 and trachea. 29 The decellularized tissues have demonstrated reduced levels of HLA panelreactive antibodies when compared with those in fresh or cryopreserved tissue.…”

“…Several recent investigations have focused on methods to avoid immunogenic reactions as well as possible infection from decellularized matrices. 26,28,33 The newly developed protocols for decellularization may help to avoid such problems and serve to create an adequate decellularized esophagus.…”

“…Human or "processed" xenograft scaffolds are almost certainly more proinflammatory when cells are disrupted and cellular debris, cytokines, and other inflammatory moieties are not thoroughly removed from the matrix. 17 Such concerns are supported by the studies of Rieder et al 16 They demonstrated that the lowest level of stimulation was with thoroughly "decellularized" human tissues. In fact, the decellularized porcine leaflets were far more attractive (stimulated macrophage response) than the extracts of human native pulmonary cusps that had not been decellularized.…”

“…16 This has been previously suggested by in vitro and in vivo animal studies and human assays. [17][18][19] The unfortunate clinical experiment with the implantation of decellularized xenograft heart valves (Synergraft) before the completion of sufficient preregulatory studies only emphasized the risk of proceeding down pathways absent full understanding of the potential for ECM provocation of the immune as well as the innate nonspecific inflammatory responses. 20 In fact, a "decellularized" xenograft scaffold may actually be more inflammatory than present versions of cryopreserved homografts (ie, with retained donor cells).…”

“…Decellularization removes the host cellular antigens, reducing the inflammatory response and minimizing immune-mediated tissue rejection [66], leaving a tubular arrangement of extracellular matrix (ECM) [67]. It is the premise that ECM proteins can integrate into the surrounding tissue [68] that makes this approach attractive.…”
