Abstract: Investigation of the relatively new technology of optical coherence tomography indicated that this imaging modality may assist the ear surgeon to assess the oval window niche intra-operatively.

“…Recently, several successful feasibility studies on optical coherence tomography (OCT) in middle ear research, both ex vivo (Pitris et al 2001;Bibas et al 2004) and in vivo (Djalilian et al 2008;Just et al 2009;Nguyen et al 2012) have prompted us to conduct preliminary tests to measure full-field thickness variations of several rabbit and human TMs with OCT . Though successful, it was clear that additional hardware and software modifications to the measurement setup were required in order to measure full membranes.…”

“…The use of OCT for surgical guidance is a recently explored concept in ophthalmology(Dayani et al , 2009) but has been used by other specialists to guide cochlear implantation,(Just et al , 2009b; Pau et al , 2007) biopsy site localization (Just et al , 2009a), and diagnosis of vocal fold lesions. (Vokes et al , 2008) Delivery systems such as hand-held and microscopic probes may enable surgical applications of OCT systems, so that a clinician can have access to cross-sectional information when performing ophthalmic surgery - as opposed to only pre- and post-operative structural data.…”

“…A separate study suggested OCT may be used to evaluate the oval window niche. (Just et al , 2009b) Vokes et al also developed a surgical microscope to enable non-contact imaging of the vocal folds, which they found may be applicable to the diagnosis of vocal fold lesions, apparent in OCT sections as a disruption of the basement membrane. (Vokes et al , 2008) Others have developed operating microscopes with integrated OCT systems and used them define a biopsy site so a resection can be precisely planned.…”

“…Several studies concerning the structure and structural changes of the TM by applying OCT were carried out in the last years [8][9][10] and recently, Rutledge et al [11] describe imaging of the internal collagen fiber structures in the lamina propria of the TM with a spatial domain OCT system. The clinical usability of OCT was demonstrated by Djalilian et al [9] (in vivo inspection of the mucosa of the tympanic cavity in order to detect cholesteatoma), Heermann et al [12] (distance measurements for middle ear prosthesis), and Just et al [13] (in depth visualization of the oval window niche). A further important application area of OCT is the measurement of motions of a sample with phase-resolved Doppler analysis [14][15][16].…”
